卵巢浆液性囊腺瘤伴低分化癌附壁结节的病理诊断及文献复习

目的复习文献,对卵巢浆液性囊腺瘤伴低分化癌附壁结节的临床病理学特点、病理诊断和鉴别诊断等进行探讨.方法对肿瘤标本做常规石蜡切片及HE染色,光镜观察,并对石蜡切片做免疫组化染色.结果患者右侧卵巢有一10cm×9cm×8cm的囊性肿块,在肿瘤内囊壁的一侧含有一个3cm×3cm×3cm的灰白色实性结节.病理诊断为卵巢浆液性囊腺瘤伴低分化癌附壁结节.结论国内文献上报道的卵巢囊性肿瘤伴附壁结节患者的卵巢囊性肿瘤大多数内粘液性肿瘤,少数为浆液性或混合性肿瘤,附壁结节可有多种组织学类型.本例浆液性囊腺瘤伴发的附壁结节的形态符合为低分化移行细胞癌,在国内文献上尚未见有报道.本文尚对卵巢囊腺瘤伴附壁结节的病理诊断、鉴别诊断及患者的预后等进行了探讨.

Review and probe on literature and diagnosis of clinical pathological features mural nodule of low differentiate carcinoma in serous cystic adenoma in ovary

Objective To review the literature and probe the clinical pathological features,pathological diagnosis and differential diagnosis of the mural nodule of low differentiate carcinoma in serous cystic adenoma in ovary. Methods The tumor was studied by using of paraffin slices,HE staining,and observed under microscope.The paraffin slices were also stained with immunohischemistrical method. Results In the operation,the patient was found that there was a 10×9×8cm cystic tumor in the right ovary with a grey mural nodule in the wall of the cystic tumor.The pathologic diagnosis was the serous cystic adenoma with a mural nodule of low differentiated carcinoma. Conclusions The most of the cystic tumors in ovaries were mucinous cystadenomas,and the minority of the cases were serous cystic adenomas or the mixture of the mucinous and serous cystic adenoma.Histologically,the mural nodules with the cystic tumors in ovary could be various kinds of the tumors.The morphologic appearance of the mural nodule in the serous cystic adenoma of our patient consisted with that of lower differential transitional cell carcinomas,and it has not been reported before in domestic literature.In this article,the diagnosis,the differential diagnoses and the prognoses of the patients of the cystic tumor with mural nodules are also discussed.
