经皮肾镜取石与输尿管碎石治疗嵌顿性输尿管上段结石的疗效比较

目的观察并比较经皮肾镜取石与输尿管碎石治疗嵌顿性输尿管上段结石临床效果及安全性。方法选取我院2009年3月至2011年5月收治嵌顿性输尿管上段结石患者203例,随机分为两组,其中对照组102例,采用输尿管碎石治疗,实验组46例,采用经皮肾镜取石治疗;治疗结束后比较两组患者手术时间、住院时间、血红蛋白下降程度、结石清除率及并发症发生率等。结果实验组患者手术时间、血红蛋白下降程度均明显高于对照组,组间比较差异有统计学意义(P<0.05);对照组与实验组患者住院时间组间比较差异无统计学意义(P>0.05);实验组患者结石清除率明显高于对照组,组间比较差异有统计学意义(P<0.05);同时对照组与实验组患者住院时间、并发症发生率组间比较差异无统计学意义(P>0.05)。结论经皮肾镜取石术治疗嵌顿性输尿管上段结石临床疗效确切,能够显著提高结石清除率,且无严重并发症,具有临床推广使用价值。

The Efficacy Comparison of Percutaneous Nephrolithotomy and Ureteroscopic Lithotripsy for the Treatment of Impacted Upper Ureteral Stones

Objective To observe and compare therapeutic effect and security ofpercutaneous nephrolithotomy and ureteroscopic lithotripsy for the treatment of impacted upper ureteral stones. Methods 203 patients with impacted upper ureteral stones were chose in the period from March 2009 to May 2011 in our hospital and randomly divided into two groups including control group （51 patients） with ureteroscopic lithotripsy treatment and experimental group （46 patients） with percutaneous nephrolithotomy treatment; and after treatment the operative time, hospital staying time, the hemoglobin decreased degree, the stone clearance rate and complication incidence were compared. Results The operative time and hospital staying time, the hemoglobin decreased degree of experimental group were significantly higher than control group （P〈0.05） . The stone clearance rate of experimental group were significantly higher than control group （P〈0.05） . And there was no significant difference in hospital staying time and complication incidence between control group and experimental group （P〉0.05） .Conclusion The percutaneous nephrolithotomy on impacted upper ureteral stones obtain excellent effect, can effectively improve the stone clearance rate, have no serious complication and is worthy of clinical application.
