腹腔灌洗与内给氧联合治疗重症急性胰腺炎并发腹腔间室综合征

目的总结腹腔灌洗与内给氧联合治疗重症急性胰腺炎(SAP)并发腹腔间室综合征(ACS)的临床经验。方法对我院2003年4月至2012年2月收治的52例患者,在各种常规治疗措施基础上,采用腹腔灌洗与内给氧联合治疗。结果顺利恢复42例(占80.8%),出现胰腺假性囊肿6例(占11.5%),腹腔感染后形成脓肿4例(占7.7%),死于多器官功能衰竭2例。结论对SAP并发ACS患者,早期采取腹腔置管引流灌洗与内给氧联合治疗,方法简单、有效、实用,有助于患者渡过高危期,值得临床推广应用。

Peritoneal Lavage and Carbamide Peroxide for the Treatment of Severe Acute Pancreatitis with Abdominal Compartment Syndrome

Objective To evaluate the clinical effect and safety of peritoneal lavage and carbamide peroxide for the treatment of severe acute pancreatitis（SAP） with abdominal compartment syndrome（ACS）.Methods From April 2003 to February 2012,Peritoneal lavage and carbamide peroxide were used to treat 52 patients beside the regular therapy.Results Forty-two of 52 patients got recovery successfully,the clinical cure rate was 80.8%.Complications included 6 patients in pancreatic pseudocyst（11.5%）,4 patients in abdominal abscess（7.7%） and the 2 remainders died of multiple organ failure.Conclusion The therapeutic methods with combining peritoneal lavage and carbamide peroxide for the treatment of SAP with ACS is simple,effective and practical.These methods can help patients to live through high-risk period,reduce the complication and mortality.
