高血压病人细胞黏附分子变化及与危险分级的关系

摘    要:①目的 观察高血压病人细胞黏附分子的变化 ,探讨其在高血压发病机制及与危险分级的关系。②方法 采用酶联免疫吸附法 ,测定 6 0例高血压病人 (低危、中危、高危、极高危组各 15例 )和 2 0例正常人白细胞CD18表达、血浆可溶性细胞间黏附分子 1(sICAM 1)和可溶性血管细胞间黏附分子 1(sVCAM 1)浓度。③结果高血压病人白细胞CD18表达、sICAM 1和sVCAM 1浓度明显增高 ,与对照组比较差异有显著性 (t =3.6 5 9~7.5 96 ,P <0 .0 1) ;白细胞CD18表达、sICAM 1和sVCAM 1浓度随高血压分级增高均呈逐渐增高的趋势 ,低危、中危、高危、极高危组各指标比较差异有显著性 (F =2 9.86~ 77.76 ,q =2 .98~ 2 0 .2 8,P <0 .0 5、0 .0 1)。高血压病人白细胞CD18表达与sICAM 1和sVCAM 1均呈正相关 (r=0 .915、0 .932 ,P <0 .0 1)。④结论 细胞黏附分子表达异常与高血压发病及危险分级有密切关系。

Abstract:Objective To investigate the changes of cell adhesion molecule in patients with hypertension and explore its value in the pathogenesis and risk grading of the disease. Methods Sixty patients were divided into four groups( 15 in each group): mild risk, moderate risk, high risk and very high risk. Enzyme linked immunosorbent assay was used to measure the plasma leukocyte CD 18 expression, soluble intercellular adhesion molecule 1(sICAM 1) and soluble vascular cell adhesion molecule 1(sVCAM 1) . The results were compared with 20 normal subjects. Results The leukocyte CD 18 expression, sICAM 1 and sVCAM 1 concentration were significantly higher in the patients than those in the controls ( t=3.659-7.596, P <0.01) and increased correspondingly with the grading, the difference was dramatically significant ( F=29.86-77.76, q=2.98- 20.28 , P < 0.05 ,0.01). The leukocyte CD 18 expression was positively correlated with sICAM 1 and sVCAM 1 ( r=0.915, 0.932 , P < 0.01 ). Conclusion The abnormal expression of cell adhesion molecule is closely related to the occurrence and risk grading of hypertension.
