活动期类风湿关节炎血脂水平变化相关性研究

摘    要:目的观察类风湿关节炎(rheumatoid arthritis,RA)患者血脂的变化及与病情活动指标的相关性。方法选取30例活动期RA患者与20例正常对照组,同时对受试者血清总胆固醇(total cholesterol,TC)、甘油三酯(trlglycerlde,TG)、高密度脂蛋白(Hish density lipoprotein,HDLC)、栽脂蛋白A(apolipoprotein Al,ApoAl)、低密度脂蛋白(Low density lipoprotein,LDLC)、载脂蛋白B(apolipoprotein B,ApoB)及活动性指标:类风湿因子(rheumatoid factor,RE)、红细胞沉降率(简称血沉,erythrocyte sedimentation rate,ESR)、C-反应蛋白(C—reactive protein,CRP)进行检测。结果与正常对照组相比,RA活动组Tc、HDLC、ApoAl显著降低(P〈0.01),ApoB显著增高(P〈0.01),而TG、LDLC无统计学意义;HDLC、ApoAl与RF、CRP、ESR呈明显负相关(P〈0.05或P〈0.01);TC与CRP、ESR呈明显负相关(P〈0.05或P〈0.01),TG、LDLC、ApoB与疾病活动性无明显相关。结论活动期RA患者出现了TC、HDLC、ApoAl显著降低,表明活动期RA患者出现了的血脂变化,而这种变化与疾病的活动性密切相关。

Correlation between Changes of Serum Lipids in Active Phase of RA and Disease Active Index

Abstract:Objective To obgerve the eorrelation between the changes of serum lipids in active phase of rheumatoid arthritis (RA) and disease active index. High density lipoprotein,HDLC. Methods 30 cases with RA in active phase and 20 normal individuals were enrolled in this study. The level of serum lipids, total cholesterol( TC), triglyceride( TG), High density lipoprotein (HDLC), apolipoprotein B (ApoB), Low density lipoprotein (LDLC), as well as active index, rheumatoid factor( RF), erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP). Results Compared with normal group, TC, HDLC and ApoA1 of RA active group decreased significantly ( P 〈 0. O1 ), ApoB of RA active group inereased gignifieantly ( P 〈 0. 01 ), while TG, LDLC had no statistical significance ; HDLC, ApoAl and RF, CRP, ESR were negatively correlated ( P 〈 0.05 or P 〈 0.01 ) ; TC and CRP, ESR were negatively correlated (P 〈 0.05 or P 〈 O. 01 ), TG, LDLC, ApoB had no obvious correlation with disease activity. Conclusion In the active phase of RA, TC, HDLC and ApoAl decrease significantly, the serum lipids of RA is changed by inflammatory factors, and this change and disease activity are closely related.
