后循环缺血患者的脑干听觉诱发电位与经颅多普勒对比分析

摘    要:目的:探讨后循环缺血患者的脑干听觉诱发电位(BAEP)及经颅多普勒(TCD)的变化并作对照分析。方法:对78例临床诊断为后循环缺血的患者进行BAEP和TCD的检测,并与健康对照组进行对比分析。结果:BAEP异常的有59例,异常率为76.0%。其中表现为脑干型35例(59.3%),混合型18例(30.5%)和内耳型6例(10.1%)。在TCD异常的55例,异常率为70.5%。主要表现在椎基底动脉系统血流速度减慢或增快;血流频谱改变及PI值增高。结论:BAEP能敏感地检出后循环缺血患者的听通道神经功能的异常,而TCD则可了解后循环缺血患者的椎基底动脉系统的血流动力异常情况,联合应用BAEP和TCD检测对后循环缺血的早期诊断、治疗和疗效观察有重要的参考价值。

Abstract:Objective:To investigate the changes of BAEP and TCD in patients with posterior circulation ischemia(PCI),and to contract analysis between them.Methods:78 patients with PCI were examined by BAEP and TCD,and 30 healthy peoples were selected as control group.Results:The abnormal rate of BAEP was 76.0%.The main abnormal finding of BAEP included the wave was poor,peak latency of Ⅰ、Ⅲ、Ⅴ waves prolonged,interpeak latency of Ⅰ-Ⅴ and Ⅲ-Ⅴ waves prolonged.The ratio of Ⅲ-Ⅴ/Ⅰ-Ⅲ was more than 1,and increased reactive threshold was noted.The abnormal rate of TCD was 76.0%.The main abnormality of TCD included decrsased or increased flow velocity of vertebral-base artery,abnormal spectrum and increase of palse index(PI).Conclusion:BAEP may detect sensitively the abnormality of hearing and brainstem junction and TCD may detect the cerebral hemodynamic abnormalities.Combination of BAEP and TCD has an important effect in the early diagnosis,therapy and prognostic evaluation.
