应用组织多普勒及血浆脑利纳肽对2型糖尿病患者左室舒张功能变化的研究

目的利用超声心动图(UCG)检测2型糖尿病(DM)患者心脏的形态结构和心功能状态,并与血浆脑钠利肽(BNP)水平相结合,评价2型DM患者左心室舒张功能的变化。方法应用脉冲多普勒超声心动图(PDE)及多普勒组织显像技术(DTI)测定了50例2型DM病人、30例健康人二尖瓣口和二尖瓣环运动频谱,定量分析BNP,结果 (1)DM组与对照组BNP水平无明显差异(P0.05)(2)DM组左心房内径(LAd)较对照组增大(P0.05);(3)反映收缩功能的指标两组比较无显著性差异,DM组反映舒张功能的多项指标异常(P0.05)。结论 2型DM患者存在舒张功能异常,DTI测定二尖瓣环运动速度评价左心室舒张功能敏感性高。

Study on Changes of Left Ventricular Diastolic Function in Patients with Type 2 Diabetes Mellitus by Uitrasonic Cardiogram and B-natriure peptide Levels

Objective To evaluate the changes of left ventricular diastolic function by detecting cardiac structure and function in patients with type 2 diabetes meilitus（DM） with echoeardiography and BNP. Methods The pulse Doppler echocardiography（PDE）and Doppler tissue irnaging（DTI）were used to examine the trans-mitral velocity, the mitral annulus velocity and BNP level in fifty patients with type 2 diabetes meilitus and thirty normal subjects,Results （1）There was no significant difference between two groups in BNP level （ P 〉 0.05）. （2） left atrial diameter increased in diabetic patients compared with those in control subjects（ P 〈 0.05）. （3） There was no significant difference between two groups in the parameters of systolic function while many parameters of diastolic function were abnormal in DM group（ P 〈 0.05）. Conclusion Diastolic dysfunction exists early in patients with type 2 diabetes mellitus. The assessment of the mitral annulus velocity by DTI is sensitive to evaluate left ventricular diastolic function.
