彩色M型多普勒和组织多普勒成像技术评价高血压患者左室舒张功能的研究

摘    要:目的:应用彩色M型多普勒(CMM)和组织多普勒成像(DTI)等方法评价高血压患者的左室舒张功能,探讨CMM和DTI评价左室舒张功能的临床价值。方法:根据舒张早期和晚期二尖瓣口血流峰值速度比值(E/A)将30例高血压患者例分为三组:高血压Ⅰ组(20例)E/A<1,和高血压Ⅱ组(10例)E/A≥1,正常对照组20例。应用CMM测量左室舒张早期血流播散速度(FPV);应用DTI分析二尖瓣环的舒张早期运动速度(Ve)、舒张晚期运动速度(Va)。结果:高血压Ⅰ组和Ⅱ组的左室舒张早期血流播散速度(FPV)、二尖瓣环舒张早期峰值速度Ea、Ve/Va与正常对照组相比均有明显下降(P<0.05)。结论:在评价左室舒张功能异常时,二尖瓣血流频谱可能出现假性正常现象;FPV、Ve、Ve/Va是综合评价左室舒张功能的有效指标。

Abstract:Objective To evaluate the left ventricular diastolic function in the patients with hypertension by color M-mode Doppler echocardiography(CMM) and Doppler Tissue Imaging(DTI).Methods Thirty essential hypertension patients were divided into two guoups according to mitral valve peak flow velocity E/A ratio: groupⅠ,E/A<1(n=20);groupⅡ,E/A≥1(n=10);twenty normal volunteers.Flow propagation velocity(FPV) in left ventricle diastole rate by CMM,mitral annular velocity in early diastole(Ve) and late diastole(Va) by DTI were measured.Results FPV,Ve,Ve /Va in hypertensive patients were lower than normal(P<0.05).Conclusions FPV,Ve,Ve /Va may be useful in distinguishing a Doppler pattern of pseudo-nomalization,which could assess diastolic function more reliably.
