血清脂蛋白a、纤维蛋白原、D-二聚体水平与急性缺血性脑卒中患者的相关分析

目的观察急性缺血性脑卒中（AIS）患者脂蛋白a（Lpa）、纤维蛋白原（Fib）、D-二聚体（DD）水平变化,并探讨其临床意义。方法对74例AIS患者,据梗死面积分为腔隙性梗死组21例,非腔隙性梗死组53例,另正常对照组34例。分别测定其血清脂蛋白a（LPa）、纤维蛋白原（Fib）和D-二聚体（DD）水平。结果（1）AIS组血清Lpa和Fib水平明显高于对照组（P〈0.01）,但两者无相关性（r=-0.067,P=0.569）;（2）DD水平明显高于对照组（P〈0.05）;（3）非腔隙性梗死组与腔隙性梗死组血清Lpa和DD比较差异有显著性（前者P〈0.01,后者P〈0.05）,而血清Fib水平比较两者无统计学意义（P〉0.05）。结论血清Lpa、Fib和DD水平在缺血性脑卒中患者中明显增高,且Lpa和DD的增高程度与梗死面积相关,证实LPa、Fib和DD是急性缺血性脑卒中临床事件的独立危险因素,对临床评估缺血性脑卒中危险及梗死面积有一定意义。

The change of the level of lipoprotein (a),fibrinogen,and D-dimer in patients with acute ischemic stroke

Objective To investigate the changes of level of lipoprotein(a)(Lpa),fibrinogen(Fib)and D-dimer(DD)and its clinical significance among in patients with acute ischemic stroke.Methods The 108 serum sample of Lpa,Fib and DD were detected.74 patients with acute ischemic stroke were entered in to AIS group;the other 34 patients without acute ischemic stroke enrolled as control group.Results There was a significant difference between two groups that the levels of Lpa,Fib,and DD in AIS group were lisher than control group,but change of level of Lpa and Fib was without correlatlon.The levels of Lpa and DD in cases with non-lacunar infarction were mavked higher than in cases with lacunar infarction(P<0.01 or P<0.05),but the results of Fib of two sub-groups have no obvious change(P>0.05).Conclusion The patients with AIS have significantly increased levels of Lpa,Fib and DD.It showed that Lpa,Fib and DD might be risk factors of AIS patients.
