新诊断2型糖尿病早期胰岛素强化治疗的临床观察

目的观察初诊2型糖尿病患者使用胰岛素强化治疗的临床疗效。方法48例初诊2型糖尿病（T2DM）患者随机分为2组，常规组（24例）采用常规降糖药治疗，强化组（24例）采用胰岛素强化治疗，治疗3个月，观察两组患者治疗前后血糖、糖化血红蛋白、胰岛素抵抗指数及胰岛β细胞功能的变化。结果强化组患者治疗后空腹血糖、餐后血糖、糖化血红蛋白、空腹胰岛素、餐后胰岛素、胰岛素抵抗指数及胰岛β细胞功能均较治疗前显著改善（P〈0．05～P〈0．01）；与对照组比较，糖化血红蛋白、空腹胰岛素、餐后胰岛素、胰岛素抵抗指数及胰岛β细胞功能改善优于对照组（P〈0．05）。结论早期进行胰岛素强化治疗能够减轻糖尿病患者胰岛素抵抗，有效改善胰岛β细胞功能。

Clinical observation of short-term intensive insulin therapy on preliminary diagnosis Type-2 diabetes mellitus

Aim To observe curative effect of short-term intensive insulin therapyon preliminary diagnosis patients of Type-2 Diabetes Mellitus. Methods 48 cases of preliminary diagnosis type-2 diabetes mellitus patients were randomly devided into two groups：routine group （ n = 24 ） treated with orally western medicine, treatment group （ n = 24 ） treated with intensive insulin therapy. After three months, the changes of FBG,PBG,HbAlc, FINS,2 h INS, Homa β and HomalR of patients in two groups before and after treatment were observed. There were significant differences in FBG, PBG, HbAlc, FINS,2hlNS, Homa β and Homa IR of treatment group between prior treatment and post treatment （ P 〈 0.05 - P 〈 0.01 ） ; Comparing with that of routine group, HbA1 c, FINS,2 h INS, Homa β and Homa I R of treatment group were improved significantly（ P 〈 0.05 ）. Conclusion Short-term intensive insulin therapy can reduce insulin resistance and effectively improve β-cell function of Type-2 Diabetes Mellitus.
