食管癌后程加速超分割放疗加同期化疗的临床疗效观察

目的比较后程加速超分割放疗加同期化疗与单纯后程加速超分割放疗治疗食管癌的疗效.方法36例中上段食管癌病人随机分为两组：后程加速超分割放疗（LCAFR）组18例,首先予前后野对穿照射,分割剂量2 GY/次,5次/周.DT40 GY后避开脊髓,分割剂量1.2 GY/次,2次/d（间隔6 h以上）,使肿瘤总量达65 GY左右.后程加速超分割放疗加同期化疗（LCAFR＋C）组18例,同LCAFR组并加用顺铂40 mg静滴,第1～3天;5-氟尿嘧啶3 g,持续泵5 d,共两周期.结果放化组和单放组的1、3 a生存率分别为83.3%、77.7%和38.9%、33.4%,但差异无统计学意义（P〉0.05）;放化组和单放组1,3 a局部控制率分别为88.9%、72.2%和61.1%、38.9%,差异无统计学意义（P〉0.05）.结论后程加速超分割放射治疗加化疗未能明显提高食管癌放射治疗的生存率,有提高食道癌的局部控制率的趋势.
