基质金属蛋白酶-2和唾液酸化Lewis-X在乳腺癌中的表达及其意义

摘    要:目的研究基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)和唾液酸化Lewis-X(sialyl Lewis-X,SLeX)蛋白在乳腺癌组织中表达及其与淋巴结转移的关系。方法应用免疫组织化学微波-链霉菌素-生物素(microwave-labelled strept-avidin biotin,MW-LSAB)法检测120例乳腺癌原发灶、71例癌转移淋巴结和30例远离癌组织的正常乳腺上皮细胞MMP-2和SLeX蛋白的表达水平。结果MMP-2和SLeX的表达在正常乳腺组织上皮细胞中阳性率分别为16.7%(5/30)和20.0%(6/30),在乳腺癌原发灶中阳性率分别为78.3%(94/120)和86.7%(104/120),两者在癌组织与正常乳腺组织中阳性率之间差异有统计学意义(P〈0.05)。在120例乳腺癌患者中,有淋巴结转移者乳腺癌原发灶MMP-2和SLeX阳性率分别为84.5%(60/71)和100.0%(71/71),无淋巴结转移者分别为69.4%(34/49)和67.3%(33/49),MMP-2差异无统计学意义(P〉0.05),SLeX差异有统计学意义(P〈0.05)。在癌转移淋巴结中,MMP-2和SLeX的阳性率分别为21.1%(15/71)和88.7%(63/71),后者与原发癌组织中的表达具有较高一致性(r=0.874,P〈0.05)。结论乳腺癌组织中MMP-2和SLeX均呈高水平表达,并且后者与淋巴结转移密切相关。作为预测乳腺癌浸润和转移的肿瘤相关抗原,联合检测对评估患者预后具有重要实用价值。
