新疆乌鲁木齐市产超广谱β-内酰胺酶肺炎克雷伯菌基因分型及耐药分析

摘    要:目的了解新疆乌鲁木齐市产超广谱β-内酰胺酶(ESBLs)肺炎克雷伯菌的基因分型和耐药性。方法采用纸片扩散法对肺炎克雷伯菌进行抗菌药物的敏感性实验,并用PCR对ESBLs进行基因型分析。结果 50株产ESBLs肺炎克雷伯菌中,有10株携带TEM型基因,有12株携带SHV型基因,有38株携带CTX-M型基因。其中,有24株同时携带两种或两种以上基因,且多为CTX-M型与TEM型或SHV型基因共同存在。另外,还有1株实验菌株扩增结果显示为阴性。50株产ESBLs的肺炎克雷伯菌对亚胺培南和美洛培南100.0%敏感;对头孢唑啉、头孢呋辛、头孢哌酮、哌拉西林100.0%耐药;对氨苄西林/舒巴坦、环丙沙星的耐药率分别为97.1%、92.2%;四环素类、磺胺类抗生素及头孢哌酮/舒巴坦耐药率分别为18.6%、28.5%及28.5%。结论新疆乌鲁木齐市产ESBLs肺炎克雷伯菌耐药性严重,基因型以blaCTX-M为主而且多基因携带情况比较常见,多为CTX-M型与TEM型或SHV型基因同时存在。

Abstract:Objective To know the super broad-spectrum beta-lactamase(ESBLs)pneumonia clay bacteria genotyped and antibiotic resistance in Urumqi City of Xinjiang.Methods The method of paper spread bacteria for pneumonia clay antimicrobial drugs sensitivity experiments,and to ESBLs PCR gene type analysis.Results 50plant production ESBLs pneumonia clay bacteria,thereare 10strains carrying TEM type gene,12strains carrying SHV type gene,there are 38strains carrying CTX-M type gene.Among them,have 24strains carrying two or more genes,and more for CTX-M type and TEM type or SHV type gene in together.In addition,there are 1strain experiment results show that the strain expansion for the negative.In the ESBLs fifty strains of bacteria pneumonia clay imipenem and los cultivates south 100.0%sensitive;To cefazolin,cefuroxime,cefoperazone and piperacillin 100.0%resistance;To ampicillin/sulbactam was,ciprofloxacin resistant were 97.1%,92.2%;Tetracycline kind,sulfa antibiotics and cefoperazone/sulbactam was the resistant were 18.6%,28.5%and 28.5%.Conclusion ESBLs pneumonia clay serious bacteria resistant in Urumqi City of Xinjiang,gene type to blaCTX-M primarily and multiple genes carry the situation is more common,many for CTX-M type and TEM type or SHV type gene also present.
