老年无症状性脑梗死危险因素及影像学临床分析

目的探讨老年无症状性脑梗死(asymptomatical cerebral infarction,ACI)的危险因素及影像学特点。方法收集180例老年缺血性脑血管病患者,根据无症状性脑梗死的定义将其分为ACI组和非ACI组,对两组的危险因素和影像学特征加以分析。结果ACI的危险因素为高血压、高血脂、糖尿病、高同型半胱氨酸血症(HHcy)、冠心病、吸烟、年龄,其中高血压、糖尿病、高血脂、HHcy与非ACI组比有显著差异(P<0.05);ACI的好发部位依次为基底节区、内囊、丘脑、放射冠、额叶、枕叶。结论ACI在老年人中常见,发病随年龄的增长而增加,并与高血压、糖尿病、高血脂、HHcy有关,影像学检查特别是头颅MRI是确诊ACI的重要条件之一。

Clinical Analysis of Risk Factors and Imageology Features for Patients with Asymptomatical Cerebral Infarction

Objective To investigate the risk factors and imageology features of asymptomatic cerebral infarction. Methods Total 180 cases elderly ischemia-cerebrovascular disease patients were divided into two groups acording to the definition of ACI:ACI group and non-ACI group.Then analyze the risk factors and imaging features. Results The risk factors of ACI are hypertension,hyperlipidemia,diabetes,high-hyperhomocysteinemia,coronary heart disease,smoking and age. Compared with non-ACI group,hypertension,diabetes,hyperlipidemia,HHcy have significant difference(P<0.05); ACI's predilection sites are followed by the basal ganglia,the internal capsule,the thalamus,corona radiata,the frontal lobe,the occipital lobe. Conclusion ACI is common in the elderly,which is correlated with hypertension,diabetes,hyperlipidemia and HHcy. The incidence of ACI is increasing with age. Imaging examination,especially head MRI is the most important condition of ACI final diagnosis.
