频谱多普勒对二尖瓣球囊瓣膜成形术47例的疗效评价

摘    要:目的 定量评价二尖瓣球囊瓣膜成形术对肺高压和心输出量的即刻和远期影响。方法 应用频谱多普勒和心导管技术测定了 47例二尖瓣球囊瓣膜成形术 (MBVP)患者术前和术后长期随访的血流动力学改变。结果 MBVP后即刻和随访期较术前心脏指数显著增加。肺动脉高压状态显著降低 (P <0 .0 1)。结论 MBVP可显著改善二尖瓣狭窄患者术后即刻和长期的血流动力学 ;频谱多普勒是无创定量评价和随访MBVP疗效的可靠技术。

Abstract:Objective Quantitatively assess the immediate and long-term effect of mitral balloon valvoplasty on severe pulmonary hypertension and cardiac output. Method Hemodynamic changes were evaluated by spectral doppler-echo and cardiac catheterization in 47 patients with severe mitral stenosis and pulmonary hypertension before, immediate after mitral balloon valvoplasty(MBVP)and at follow-up. Result It was revealed that cardiac indexes increased significantly and pulmonary hypertension reduced markedly after MBVP and at follow-up(P<0.01). Conclusion MBVP can improve significantly the immediate and long -term hemodynamic status in patients with mitral stenosis after MBVP; spectral Doppler-echo is a reliable technique in the noninvasive quantification of the short and long-term effect of MBVP.
