速度向量成像技术评价阿霉素致兔早期左心室径向收缩功能损害与病理对照研究

摘    要:目的探讨速度向量成像(VVI)技术评价阿霉素致兔早期左心室径向收缩功能损害的价值,并与病理结果对照。方法将41只新西兰纯种兔随机分成4组:A组9只,作为对照组,每周1次注射生理盐水2ml/kg,共8周;B组10只,每周1次注射阿霉素2mg/kg,注射2周;C组10只,给予相同剂量阿霉素4周;D组12只,给予相同剂量阿毒素8周。每组于停药后2周对兔心脏行常规超声心动图及左心室各节段径向运动速度、应变、应变率检测。所有实验动物在完成超声检查后即刻处死,取出心脏,行病理学检查。结果各组同一节段径向运动各参数测量:与A组比较,B、C、D组的速度、应变及应变率均无改变。左心室射血分数及左心室缩短率D组较A组减低(P〈0.05),B、c组无改变。病理结果显示从B组到D组心肌损害逐渐加重。结论径向VVI参数与纵向VVI参数在阿霉素致兔心肌损害过程中的不同表现,有助于进一步了解心肌损害的动态发展过程。

Abstract:Objective To evaluate the experimental value of velocity vector imaging (VVI) in early assessment of left ventricular radial systolic function in rabbits with adriamycin-induced myocardial damage. Method A total of 41 rabbits were classified into four groups (9,10,10 and 12 rabbits in groups A,B,C and D respectively). Adrimycin was administrated with 2 mg/kg once week from 2-8 weeks at Group B, C and D. The control group of 9 rabbits was administrated with normal saline once a week for 8 weeks. After 2 weeks of injection, echocardiograph and velocity, strain, strain rate were measured. Histological examination were completed for all rabbits after sacrificed. Results The strain, strain rate, velocity from A, B, C, D groups were of no statistical significance( P 〉 0.05 ). LVEF and LVFS from group D were significantly lower than those in group A ( P 〈 0.05 ). The pathological examination demonstrated that the damage severity was progressively increased from group B to C to D. Conclusions The difference VVI between radial and longi- tudinal contractility was helpful for our understanding of adriamycin-induced myocardial damage.
