高频超声评价膝关节骨关节炎半月板突出

目的 采用高频超声和X线平片检查,探讨膝关节骨关节炎半月板突出与骨关节炎严重程度的相关性.方法 选择50例临床诊断骨关节炎患者,共71个有症状膝关节(骨关节炎组).另选择年龄匹配的10名无膝关节症状的健康志愿者作为对照组,共15个膝关节.骨关节炎组与对照组均接受高频超声及X线平片检查,超声观察内侧半月板突出的程度;同时对所有膝关节平片进行KL评分,并将内侧半月板突出程度同KL评分进行对照分析.结果 骨关节炎组71个膝关节中,高频超声检出52个膝关节存在内侧半月板突出,其中轻度突出17个,中度突出24个,重度突出11个.骨关节炎组所有X线检查均有异常,其中12个膝关节KL评分为1;26个为2;21个为3,12个为4.对照组中,高频超声检出3个膝关节有内侧半月板突出,均为轻度突出;其中2个KL评分为1,其余X线表现正常.内侧半月板突出的发生率骨关节炎组明显高于对照组(χ2=15.23,P＜0.001).内侧半月板突出程度同KL评分存在线性相关(χ2=27.01,P＜0.001).结论 内侧半月板突出是膝关节骨关节炎患者超声表现中的一个重要征象,并与骨关节炎的平片分级相关.

High frequency ultrasonographic evaluation of meniscal extrusion in patients with knee osteoarthritis
