探讨巨大儿对分娩的影响及产前预测

目的分析巨大儿对分娩的影响及产前预测。方法选择2011年1月1日—2011年12月31日于该院分娩的巨大儿248例作为观察组,同期248例出生新生儿正常体质量儿组作为对照组,分析巨大儿对分娩方式和母婴并发症的影响。结果观察组自然分娩率为25.81%,显著低于对照组的62.50%（P〈0.05）;剖宫产率和产后出血率分别为66.13%、9.68%,显著高于对照组的32.66%、2.02%（P〈0.05）;巨大儿经阴道分娩发生会阴Ⅲ度撕裂、新生儿窒息、新生儿产伤、肩难产率显著高于剖宫产（P〈0.05）。结论巨大儿是头位难产的主要原因之一,剖宫产率明显增加,产妇及围产儿并发症增加,因此产前预测巨大儿,选择合理分娩方式,适当放宽剖宫产指征,可减少母婴并发症的发生率,提高优生水平。

Discuss the influences of macrosomia for delivery and prenatal prediction

Objective To analyze the effect of macrosomia for delivery and prenatal prediction Methods 248 cases of tremendous children in our hospital between January 2011 and December 2011 were collected as the observation group, 248 cases of normal birth weight newborns in the same period were collected as the control group, the impact on mode of delivery and maternal complications of tremendous children were analyzed.Results The natural birth rate of observation group was 25.81%, significantly lower than 62.50 % in the control group（P〈0.05）;The cesarean section rate and postpartum hemorrhage rate was 66.13% and 9.68%,significantly higher than the 32.66%, 2.02% of control group（P〈0.05）; Ⅲ degree perineal tear, neonatal asphyxia, neonatal birth trauma, shoulder dystocia rate of observation group was significantly higher than the control group（P〈0.05）. Conclusion Macrosomia is one of the head of the main causes of dystocia, cesarean section rate increased significantly, maternal and perinatal complications increases, so the prenatal predicting macrosomia, choose reasonable way of delivery, relax appropriately cesarean section indications, can decrease the incidence of maternal and infant complications, improve the level of healthy birth.
