依托咪酯复合芬太尼在小儿全身麻醉中的临床应用

目的探讨依托咪酯复合芬太尼在小儿全身麻醉中的临床应用,评估其效果及安全性。方法选择该院小儿外科于2011年2月—2012年2月收治的66例择期施行全麻手术的患儿,按照麻醉药物不同分为丙泊酚复合氯胺酮麻醉组（A组）和依托咪酯复合芬太尼麻醉组（B组）,分别记录两组间患儿特定时间点的心率、平均动脉压、血氧饱和度及苏醒时间、并发症发生率并进行比较。结果麻醉前两组患儿的心率、平均动脉压、血氧饱和度无明显差异（P〉0.05）,麻醉后B组患儿各时间点的心率、平均动脉压明显低于A组（P〈0.05）,麻醉苏醒时间较A组短（P〈0.05）,相关并发症发生率亦低于A组（P〈0.05）。结论在小儿全身麻醉手术中,依托咪酯复合芬太尼麻醉能有效地维持机体血流动力学的稳定,缩短患儿术后复苏时间,且未出现低血压、心动过缓、躁动及恶心呕吐等相关并发症;与传统的麻醉方法相比,具有更加高效、安全的优势,可在临床上推广应用。

The clinical application of etomidate combined with fentanyl in general anesthesia with children

Objective To assess the clinical application of etomidate combined with fentanyl in general anesthesia with children,evaluate the effect and safety. Methods A total of 66 operation with general anesthesia in children from 2011 February to 2012 February were divided into propofol combined with ketamine group（A group）and etomidate combined with fentanyl group（B group）.To record and compare the difference of heart rate, mean arterial pressure and blood oxygen saturation in specific time and postoperative recovery time, complication rate between the groups. Results There had no obvious difference in heart rate, mean arterial pressure and blood oxygen saturation between the groups before general anesthesia（P〉0.05）. After general anesthesia, the heart rate, mean arterial pressure in specific time with B group was lower than that with A group（P〈0.05）, postoperative recovery time was shorter than that with A group（P〈0.05）, and complication rate was lower than that with A group（P〈0.05）. Conclusion Compare with the traditional way of anesthesia in children, etomidate combined with fentanyl in general anesthesia provides stability of hemodynamics, shorter postoperative recovery time, and no complications such as hypotension, bradycardia, dysphoria,nausea and vomiting. So the clinical application of etomidate combined with fentanyl in general anesthesia could be recommended for the efficient and safety.
