30例闭合性腹部急性损伤的早期诊断与治疗

目的探讨30例闭合性腹部急性损伤的早期诊断与治疗。方法收集该科诊断为闭合性腹部急性损伤的患者,收集时间为2008年1月—2013年3月,共计30例,随机分为3组,单独使用B超单独检查,为B超组10例;单独使用腹部CT单独检查,为CT组10例,联合B超和腹部CT检查,为联合组10例。对3组患者的诊断准确率及手术、住院时间进行比较。结果 B超组、CT组、联合组诊断准确率分别为60%、80%、100%,3组诊断准确率差异有统计学意义（P〈0.05）;B超组、CT组、联合组患者治疗手术时间分别为（9.7±2.4）h、（8.4±3.6）h、（4.2±2.7）h,3组手术时间差异有统计学意义（P〈0.05）,3组住院时间分别为（12.3±1.4）、（10.1±1.3）、（7.5±1.1）d,3组住院时间差异有统计学意义（P〈0.05）。结论与B超组与CT组单独比较,B超联合腹部CT治疗闭合性腹部急性损伤准确率最高,为手术前的定位,定性诊断提供了最优的帮组,手术时间最短,住院时间最短。

30 cases of acute blunt abdominal injury in the early diagnosis and treatment

Objective Study of 30 cases of acute blunt abdominal injury in the early diagnosis and treatment.Methods Collected for the diagnosis of acute blunt abdominal injury patients, collect in time for the 2008 January to 2013 March, a total of 30 cases,were randomly divided into 3 groups, the use of B ultrasound examination alone alone, as B ultrasound group 10 cases; the use of abdominal CT examination alone alone, for 10 cases in group CT, the United B ultrasound and abdominal CT examination, as the joint group of 10 cases. Diagnosis of three groups of patients with accuracy and operation, duration of hospitalization were compared.Results B ultrasound group, CT group, combined group, the accuracy rate of diagnosis were 60%, 80%, 100%, three sets of diagnostic accuracy rate difference was statistically significant（P〈0.05）;B ultrasound group, CT group, combined group patients operation time was（9.7±2.4）h、（8.4±3.6）h、（4.2±2.7）h, three groups had statistical significance operation time difference between the three groups（P〈0.05）, the hospitalization time was（12.3±1.4）、（10.1±1.3）、（7.5±1.1）d, the three groups had statistical significance（P〈0.05） hospitalization time difference.Conclusion Compared with the B ultrasound alone group and CT group, acute injury B ultrasound combined with abdominal CT in the treatment of closed abdominal accuracy rate, as the location before operation,qualitative diagnosis provides optimal group, operation time, hospitalization time.
