| 摘 要： | 该文报道了5例脾功能亢进患者行腹腔镜脾切除术的初步体会。从1996年4月～1997年3月，行腹腔镜脾切除术5例，全部获成功。平均手术时间为3．8h（2．8～5h），平均水中失血650ml（50～1200ml），术后翌日恢复流质饮食，无术后并发症，平均水后住院7d（5～10d）。腹腔镜脾切除术具手术创伤小和恢复快的优点，但其手术时间较长和水中失血较多。腹腔镜牌切除术可适用于需脾切除的血液病患者。推荐左前侧厦里手术径路。

| Abstract: | From April 1996 to March 1997, five cases of hypersplenism were successfully treated by laparoscopic splenectomy. No conversions from laparoscopic to open method were neccessary. The operations averaged 3. 8 hours (range 2.8~ 5 hours) and the mean blood loss wos 650mI (range 50 ~ 1200ml). Patients tolerated an liquid diet on the first postoperative day, and no post-operative complications were observed. The median post-operative hospital stay was seven days (range 5 ~ 10 days). Laparoscopic splenectomy has minimal incisions, quick recovery time, and a longer operation time. Laparoscopic splenectomy can be used as a surgical treatment of hematologic diseases. We recommend the antero-lateral approach when performing laparoscopic splenectomy. |
