骨髓转移癌的临床分析

【摘要】 目的 探讨骨髓转移癌的临床特点。方法〓对2002年至2014年由中山大学附属第一、第三医院、中山大学孙逸仙纪念医院收集到具有完整资料的156例骨髓转移癌患者临床资料进行临床分析。结果〓156例骨髓转移癌患者平均年龄61岁,其中贫血(121例,77.6%)及全身骨骼疼痛(98例,62.8%)为最常见症状。生化检查显示血清碱性磷酸酶升高97例(62.2%),乳酸脱氢酶升高72例(46.2%)。骨髓象发现骨髓增生活跃或明显活跃108例,骨髓增生减低或重度减低22例。预后差,生存期大多不超过1年。总共119例(76.3%)患者找到原发病灶。结论〓骨髓转移癌通常有贫血及全身骨骼疼痛等临床症状,容易出现外周血清碱性磷酸酶及乳酸脱氢酶升高,患者预后较差,需要早期发现治疗。

Preliminary clinical analysis of bone marrow metastasis

Objective To investigate the clinical characteristics of bone marrow metastasis. Methods The complete information of 156 cases with bone marrow metastasis was collected from the First Affiliated Hospital of Sun Yat-sen Memorial Hospital and the First Affiliated Hospital and the Third Affiliated Hospital of Sun Yat-sen Hospital from 2002 to 2014. Results The average age of the 156 cases was 61 year old. The most common symptoms were anemia (121 cases,77.6%) and bone pain (98 cases,62.8%). Biochemical examination showed raised levels of serum alkaline phosphate (97 cases,62.2%) and lactate dehydrogenase (72 cases,46.2%). 108 cases showed hyperplasia of bone marrow, and 22 cases presented reduced proliferation. Most cases could survive less than one year. A total of 119 (76.3%) cases with metastasis tumors were found premary sites. Conclusion In the present group of patients with bone marrow metastasis, most usually appeared anemia and bone pain, alkaline phosphate and lactate dehydrogenase were often increased. Patients always had poor prognosis and needed to be diagnosis early.
