支气管哮喘患儿血清瘦素水平与临床严重程度的关系

摘    要:目的:探讨支气管哮喘患儿血清瘦素水平与临床严重程度的关系。方法:将支气管哮喘患儿依据病情严重程度分为轻度间歇组(A组)、轻度持续组(B组)及中度持续组(C组),比较各组间及组内治疗前后血清瘦素水平,并对瘦素和身体质量指数(body mass index,BMI)进行相关性分析。结果:A组治疗前后血清瘦素水平比较差异无统计学意义(P〉0.05);B、C组治疗后血清瘦素水平均低于治疗前(P〈0.05)。A、B、C三组治疗前血清瘦素水平依次升高(P〈0.05),治疗后A、B、C三组血清瘦素水平比较差异均无统计学意义(P〉0.05);治疗前血清瘦素水平与BMI无相关性(r=-0.061,P〉0.05)。结论:哮喘患儿血清瘦素水平与病情严重程度相关,其原因可能与瘦素介导炎症细胞活化和炎症介质释放的功能有关,具体机制尚需进一步研究。

Abstract:Objective: To investigate the association between the clinical severity of childhood asthma and serum leptin levels. Method: Subjects were distributed into three groups according to the severity of asthma,including mild intermittent group( group A),mild persistent group( group B),moderate persistent group( group C). Serum leptin levels were detected before and after treatment,then intragroup comparisons and comparisons between groups were analyzed. Correlations of leptin with body mass index( BMI) were analyzed. Result: Leptin levels in group A had no significant changes before and after treatment( P 0. 05); leptin levels in group B and group C after treatment were lower than these before treatment( P 0. 05); leptin levels measured before treatment gradually reduced according to the sequence of group C,B and group A( P 0. 05); leptin levels measured after treatment in three groups had no significant changes. Serum leptin levels before treatment showed no obvious correlations with BMI. Conclusion: Leptin levels correlated with the clinical severity of asthma and this was suggested to be associated with the severity of inflammation in asthma. More studies are required to evaluate leptin levels in predicting asthma severity and estimating treatment effect.
