地佐辛应用于急诊饱胃患者清醒插管的安全性研究

目的：研究地佐辛用于急诊饱胃患者清醒插管的安全性,评价其对血流动力学的影响及其有效性。方法：选取我院2012-05-2013-05间的急诊饱胃患者手术病例66例,采用随机数表法随机分成实验组和对照组,每组33例;实验组地佐辛0.1mg/kg,对照组等体积的生理盐水。比较两组患者入室时（T1）、一次表麻（T2）、二次表麻（T3）、插管时（T4）时的SBP、DBP和HR的变化及插管时的呛咳反应率、呕吐发生率、返流发生率和手术后12h随访的患者对插管过程的满意度。结果：实验组从T1到T4的各个时间点SBP、DBP、HR的监测值波动较小,而对照组的波动较大;两组患者插管后呛咳反应率、呕吐发生率、返流发生率和术后l2h随访满意度比较显示差异有统计学意义（P〈0.05）。结论：地佐辛的应用使得患者在插管前保持清醒及无呼吸抑制,气管插管时血流动力学也更平稳。

The safety of dezocine applied in emergency awake intubation for patient with full stomach

Objective：To investigate the safety of dezocine applied in emergency awake intubation for patient with full stomach,and its effects on hemodynamics. Method： 66 patients with full stomach accepted emergency op eration from May 2012 to May 2013 in our hospital were enrolled in the study and were randomly divided into ex perimental group and control group using a random number table method, each group has 33 cases. Experimental group received dezocine 0.1 mg/kg. An equal volume of saline was applied in control group. The changes of SBP, DBP,and HR,the cough response during intubation, the incidence of emesis, reflux rate and the postoperative pa- tient＇s satisfaction were compared during loading（T1 ）, 1 st topical anesthesia （T2）, 2nd topical anesthesia （T3）, and incubation （T4）. Result： SBP.DBP and HR were less volatile in experimental group at each point from T1 to T4, while they fluctuated greatly in the control group. The cough response during incubation, the incidence of emesis, reflux rate and the postoperative patient＇s satisfaction had statistically significance（P〈0.05）. Conclusion： Dezocine application allowed patients to stay awake before intubation with no respiratory depression,and the hemodynamic were more stable during intuhation.
