乳腺癌MRI形态学表现与C-erbB-2表达的相关性研究

目的　探讨乳腺癌MRI形态学表现与C erbB 2癌基因的相关性。方法　对 78例乳腺癌和 42例乳腺纤维腺瘤患者术前行乳腺MR扫描 ,分析乳腺癌的MRI形态学表现。术后标本行免疫组织化学染色测定肿瘤细胞C erbB 2的表达情况 ,并分析与相应病灶MRI表现的关系。结果　乳腺癌MRI多表现为边缘形态不规则或毛刺状边缘、边界模糊、瘤内有坏死。乳腺癌内部坏死与癌细胞C erbB 2阳性表达呈显著正相关 (r=0 .2 7,P <0 .0 5 ) ;而边界模糊、毛刺状边缘与癌细胞C erbB 2表达情况无显著相关性 (r =0 .11,r=-0 .13 ,P >0 .0 5 )。结论　乳腺癌MRI形态学表现与分子生物学指标C erbB 2之间存在一定的相关性。乳腺癌的MRI表现从一定程度上反映了癌细胞的生物学行为和预后 ,为临床治疗方案的选择提供客观依据。

Relation of MRI morphological features and the expression of C-erbB-2 in breast cancer

Objective To investigate the correlativity between magnetic resonance imaging (MRI) morphological features and the expression of C erbB 2 in breast cancer. Methods MR scanning was performed on 78 patients with breast cancer and 42 patients with fibroadenoma before operation. The MRI morphological features of breast cancer were analyzed. C erbB 2 expression of the specimens of the patients with breast cancer and fibroadenoma after mastectomy were determined by immunohistochemistry. The relationship between the immunohistochemical results and the MRI morphological features was analyzed. Results Irregular shape or spiculated margin, obscure margin and necrotic center were found more frequently on plain MRI in patients with breast cancer. C erbB 2 positive expression was positively correlated with necrotic center of breast cancer ( P <0.05) but the expression of C erbB 2 was not significantly correlated with spiculated margin and obscure margin of breast cancer ( P >0.05). Conclusion There is correlation between MRI morphological features and the expression of C erbB 2 of breast cancer to a certain degree. MRI features can reflect the biologic behavior and prognosis of breast cancer, which can provide evidence for clinical treatment planning of breast cancer.
