阿立哌唑与氯氮平治疗难治性精神分裂症对照研究

目的探讨阿立哌唑治疗难治性精神分裂症的疗效及安全性。方法将68例难治性精神分裂症患者随机分为两组各34例,分别给予阿立哌唑及氯氮平治疗。疗程8w。于治疗前及治疗第2w、4w、8w末采用潘氏量表评定临床疗效、副反应量表评定不良反应。结果治疗8w末,研究组显效率44.1%,有效率76.5%;对照组分别为52.9%,85.3%,两组总体疗效相当(P均>0.05)。潘氏量表评分两组总分、一般精神病理因子分于治疗2w末起均较治疗前有极显著性下降,阳性症状、阴性症状因子分治疗4w末起均较治疗前有极显著性下降(P均<0.01),两组随治疗时间的延续均呈持续性下降。研究组不良反应发生率较低,程度较轻。结论阿立哌唑治疗难治性精神分裂症疗效与氯氮平相当,且安全性高,依从性好,值得临床推广应用。

A control study on aripiprazole vs. clozapine in the treatment of refractory schizophrenia

Objective To explore the curative effects and safety of aripiprazole in refractory schizophrenia.Methods 68 patients with refractory schizophrenia were randomly divided into experimental(n=34,receiving aripiprazole)and control group(n=34,did clozapine)for 8 weeks.The clinical efficacies were assessed with the Positive and Negative Syndrome Scale(PANSS)and adverse effects with the Treatment Emergent Symptom Scale(TESS)before treatment and at the end sof the 2nd,4th and 8th week treatment.Results The rates of excellence and incidence of aripiprazole were 44.1% and 76.55 and those of clozapine 52.9% and 85.3% respectively,efficacies of both 2 groups were equivalent(P>0.05).Compared with pretreatment,the total and general psychopathological factor score of the PANSS lowered very more significantly since the end of the 2nd week and those of positive and negative symptoms did since the end of the 4th week of both groups(P<0.01),so did along with treatment lasting.The incidence and degree of adverse effects of aripiprazole was lower and milder.Conclusion Aripiprazole has equivalent efficacy to clozapine,higher safety and better compliance in the treatment of refratory schizophrenia.
