经脐单孔腹腔镜阑尾切除术38例

目的探讨完全经脐单孔腹腔镜阑尾切除术的可行性及应用前景。方法选取2009年6月-2010年6月我科收治的阑尾炎患者108例，其中经脐单孔腹腔镜阑尾切除术38例，开腹阑尾切除术70例。比较两种术式的手术时间、下床活动时间、术后排气时间、镇痛药使用率、手术切口感染率、残余脓肿发生率、置管引流率和住院时间。结果除手术时间、残余脓肿发生率和综合费用外，腹腔镜组和开腹组以上各指标差异均有统计学意义（P〈0．05），腹腔镜手术组有明显的优势。结论完全经脐单孔腹腔镜阑尾切除术在技术上安全可行。

Analysis of Totally Transumbilical Single-port Laparoscopic Appendectomy

Objective To evaluate the clinical feasibility and applying prospect of totally transumbilical single-port laparoscopic appendectomy. Methods Thirty eight patients underwent single-port laparoseop!c appendectomy and seventy cases with open appendectomy （ OA ） were selected in our department from June 2009 to June 2010. The operation time, use of pain-killer, complications, and the period and cost for hospitalization were studied. Results There were significant differences between transumbilical single-port laparoscopic appendectomy and OA among these indexes （ except the operation time, the incidence of residual abscess and the cost for hospitalization ） （P 〈 0.05 ）. Conclusion Totally transumbilical single-port laparoscopic appendectomy is technically feasible and safe.
