降纤酶治疗急性脑梗死60例疗效观察

摘    要:目的观察降纤酶在治疗急性脑梗死的疗效。方法选择60例急性脑梗死患者,随机分为两组:治疗组(33例)给予降纤酶10、5、5U,各加生理盐水250mL于入院第1、3、5天静滴。其余治疗同对照组(27例),即常规脱水、扩管、改善微循环等。观察两组治疗前后神经功能缺损评分、纤维蛋白原、血粘度、凝血酶原时间和活动度及肝肾功能的变化。结果治疗组有效率、显效率(87.87%和69.69%)明显高于对照组(63.7%和56.5%),P<0.05。治疗组血浆纤维蛋白原水平显著降低与对照组差异有统计学意义(P<0.01)。两组治疗后血液粘度、血浆粘度均有显著性降低,组间比较血液粘度的差异有统计学意义(P<0.05)。治疗前后两组凝血酶原活动度、凝血酶原时间的比较及谷丙转氨酶和尿素氮均无统计学意义(P>0.05)。结论应用降纤酶治疗急性脑梗死具有较高的安全性和疗效。

Abstract:Objective To evaluate the efficacy and safety of defibrase in acute cerebral infarction (ACI). Methods Sixty patients with ACI were randomly divided into defibrase group (33 cases) and no defibrase group (27 cases). Defi brase group was given defibrase 10U,5U,5U added to 0.9% NS 250mL ivd on day 1, 3, 5 of hospitalization. All patients were treated with deaquation, expanding b lood vessel and improving microcirculation. The variance of neurological impairm ent , FIB, BV, PT, liver function and renal function were evaluated in the two g roups. Results The effective rate and significant effective rate of defibrase group(87.87% vs 69.69%)were higher than that of no defibra s e group(63.7% vs 56.5%)( P<0.05 ). Fibrinogen of defibrase group was low er than that of no defibrase group. After treatment, BV and PV in the two groups were significant lower than pretherapy. In group comparison, BV was significant difference (P<0.05). PA, PT, GPT and BUN were no difference between pret h erapy and post-treatment(P>0.05). Conclusion Defibra se is an effectual and safe drug to treat acute cerebral infarction.
