糖尿病钙代谢指标的变化及其临床意义

本文观察了91例糖尿病患者6项钙代谢指标的变化,并按病程、控制状况等不同情况分组观察对钙代谢指标的影响。结果表明,糖尿病患者有钙代谢异常,主要表现为AKP明显升高伴细胞内Ca降低、尿Ca排出增加而细胞外Ca变动不大。糖尿病控制差时钙代谢异常表现为细胞外总ca值升高、细胞内Ca值降低及AKP升高。认为淋巴细胞内Ca水平可能是反映糖尿病钙代谢紊乱的一个较好指标。

THE CHANGES OF CALCIUM METABOLISM IN DIABETES MELLITUS AND ITS CLINICAL SIGNIFICANCE

Six parameters of calcium metabolism - ICa, TCa, ICa / TCa, AKP, IntraCa (intracellular calcium), and UCa (urinary calcium) in 91 cases with diabetes mellitus (DM) were measured and compared with those in normal subjects. It could be seen that significant differences existed between the two groups in AKP, IntraCa and UCa (P<0.001, 0.05 respectively), but not in ICa, TCa and ICa / TCa. IntraCa showed negative correlation with GHG. The degree of disease control influenced the IntraCa level markedly (F=4.70, P<0.05). FPG measured at the same time bore significant relation with serum AKP level (F=8.44, P<0.01) and the same was found between the type, cause of DM and UCa. The results indicate that: 1) IntraCa reflects the disturbances of calcium metabolism in DM better than ICa and TCa; 2) The excretion of calcium in urine is most obvious in the earlier stage of the disease; 3) Severe decrease of IntraCa may be one of the factors which caused a series of cellular metabolic disorders.
