HBeAg阴性慢性乙型肝炎患者相同肝实质细胞体积分摊的血清HBV DNA载量与肝组织炎性分级的关系

摘    要:目的研究乙型肝炎e抗原(HBeAg)阴性慢性乙型肝炎(CHB)患者由相同肝实质细胞体积分摊的血清HBV DNA载量与肝组织炎性分级(G)的关系。方法采用荧光多聚酶链反应检测118例HBeAg阴性CHB患者的血清HBV DNA载量,通过推算肝实质细胞体积获得不同炎性分级患者相同肝实质细胞体积分摊的HBV DNA载量。多组间资料的比较采用ANOVA检验,不同肝组织炎性分级与患者年龄、血清HBV DNA定量及相同肝实质细胞体积分摊的HBV DNA载量的相关性分析采用Pearson’s分析。结果118例HBeAg阴性CFIB患者中,肝组织炎性分级为1、2、3和4级的患者血清HBV DNA载量分别为2.75×10^5±5.73×10、3.12×10^5±2.50×10、6.03×10^5±3.02×10、7.40×10^5±2.81×10,各级之间比较差异无统计学意义(G1与G2、G3、G4比较,t值分别为0.41、0.39和0.49,G2与G3、G4比较,t值分别为0.36和0.46,G3与G4比较,t值为0.45,P值均〉0.05);而各炎性分级患者由相同肝脏实质细胞体积分摊后的血清HBV DNA载量分别为2.18×10^8±5.78×10^2、7.45×10^8±1.58×10^2、8.87×10^9±6.25×10^2、7.33×10^10±5.68×10^2,其中G3与G1,Gg与G1以及G4与G2之间比较差异有统计学意义(t值分别为0.67、0.82和0.77,P值均〈0.05)。相关性分析显示,肝组织炎性分级与相同肝实质细胞体积分摊的HBV DNA载量呈正相关(r=0.308,P〈0.05),而与年龄和血清HBV DNA载量无相关性(r值分别为0.121和0.111,P值均〉0.05)。结论HBeAg阴性CHB患者肝组织炎性程度与相同肝实质细胞体积分摊的血清HBV DNA载量水平呈正相关。

Serum HBV DNA loads apportioned by the same hepatic parenchyma cell volume and the liver inflammation grades in HBeAg-negative chronic hepatitis B

Abstract:Objective To investigate the relationship between serum HBV DNA loads apportioned by the same hepatic parenchyma cell volume and the liver inflammation grades (G) in hepatitis B e antigen (HBeAg)-negative chronic hepatitis B (CHB). Methods Serum HBV DNA loads in 118 HBeAg-negative CHB patients were detected by fluorescence polymerase chain reaction. Serum HBV DNA loads apportioned by the same hepatic parenchyma cell volume in patients with different liver inflammation grades were calculated. ANOVA (analysis of variance) was used to compare the measurement data among the groups, and the correlations of liver inflammation grads with ages, serum HBV DNA loads and that apportioned by the same hepatie parenchyma eell volume were analyzed by Pearson' s correlation test. Results Serum HBV DNA loads in liver inflammation grade 1,2, 3 and 4 were 2. 75 × 10^5 ± 5.73 × 10, 3.12 × 10^5 ±2. 50 × 10, 6. 03 ×10^5 ±3.02 × 10 and 7.40 ×10^5 ±2. 81 ×10 in 118 patients with HBeAg-negative CHB,respectively. No statistical difference was observed among four liver inflammation grades ( G1 vs. G2 : t = 0.41, G1 vs. G3: t=0.39, G1 vs. G4: t=0.49, G2 vs. G3: t=0.36, G2 vs. G4: t=0.46, G3 vs. G4 : t = 0.45, P 〉 0. 05 ). Serum HBV DNA loads apportioned by the same hepatic parenchyma cell volume in their located fibrosis stage were 2.18 × 10^8 ±5.78 × 10^2 , 7.45 × 10^8 ±1.58 × 10^2 , 8.87 × 10^9 ±6.25 × 10^2 and 7.33 × 10^10 ±5.68 × 10^2 , respectively. Statistical differences were observed between G3 and G1, G4 and G1, G4 and G2 ( t = 0.67, 0. 82 and 0.77, P 〈 0.05 ). Liver inflammation grades were positively correlated with serum HBV DNA loads apportioned by the same hepatic parenchyma cell volume ( r = 0. 308, P〈0.05), but not with the ages and the serum HBV DNA loads (r =0. 121 and 0. 111, P 〉0.05). Conclusion Serum HBV DNA load apportioned by the same hepatic parenchyma cell volume is positively correlated with liver inflammation grades in HBeAg-negative CHB.
