分娩活跃期应用安定对产妇及新生儿的影响

目的:分析经阴道分娩产妇在产程进入活跃期静脉注射安定对产程进展及新生儿的影响。方法:选取2009年2月～2010年2月分娩的初产妇240例,随机分为为治疗组及对照组各120例,治疗组在人工破膜的基础上,静脉缓慢注入安定10mg,严密观察产程进展并对产妇宫颈扩张率、新生儿Ap-gar评分、羊水污染、产后出血量等情况进行观察。对照组产妇采用阴道分娩常规处理,分娩过程中不使用安定。结果:治疗组产程活跃期(尤其活跃晚期)静脉缓慢注射安定可加速宫颈口扩张,缩短产程,疗效确切(P<0.05),但新生儿窒息发生率、产后出血较对照组高(P<0.05)。结论:产程活跃期静脉注射安定可明显缩短产程,并加速宫颈口扩张,但要积极预防新生儿窒息及产后出血。

Effect of diazepam on puerpera and neonates during delivery active phase

Objective: To assess the effect of diazepam on process of labor of puerpera with vaginal delivery and neonates during delivery active phase.Methods: A total of 240 puerperae admitted from February 2009 to February 2010 were selected,and randomly divided into treatment group and control group by half.Besides artificial rupture of fetal membranes,patients in treatment group were treated by the injection of diazepam(10 mg,i.v.).The degrees of cervix dilation,Apgar score of newborns,incidence of pollution of amniotic fluid,postpartum blood loss were observed.Patients in control group underwent conventional vaginal delivery and no diazepam was applied in the whole process.Results:The injection of diazepam(i.v.) during active phase of labor(late active phase,especially) could significantly speed up cervical dilation,and shorten the labor process(P<0.05),while the incidence of asphyxia neonatorum,postpartum blood loss were significantly higher than these of control group(P<0.05).Conclusions:Injection of diazepam(i.v.) during active phase of labor can shorten the labor process and speed up the cervical dilation significantly.But strenuous efforts should be made to prevent asphyxia neonatorum and postpartum blood loss.
