摘    要:目的:探讨1型糖尿病(T1DM)患儿血清25羟维生素D325(OH)D3]水平及其与临床常用相关指标糖化血红蛋白(HbA1C)、空腹C肽、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)、总胆固醇(TC)等生化指标的相关性。方法:对新发的26例T1DM患儿(T1DM组)、24例健康儿童(对照组)用全自动生化分析仪检测空腹血糖(FBG)、TC、TG、HDL-C、LDL-C、Ca、载脂蛋白(Apo)、脂蛋白A(Lp-A)等指标;用Bio-Rad糖化血红蛋白检测仪(高效液相色谱法)检测HbA1C;用电化学发光法检测空腹C肽水平;用酶联免疫吸附(ELISA)法测定血清25(OH)D3,并分析其相关性。结果:两组TC、LDL-C、Apo-A、Apo-B、Lp-A、Ca水平差异无统计学意义(P>0.05),T1DM组TG、ALP、HbA1C水平高于对照组,差异有统计学意义(P<0.01),T1DM组25(OH)D3、HDL-C、空腹C肽水平均低于对照组,差异有统计学意义(P<0.05~0.01);血清25(OH)D3浓度与TG、HbA1C、空腹C肽呈负相关(P<0.05~0.01),与HDL-C呈正相关(P<0.01),与钙离子、TC无明显相关性。结论:维生素D与T1DM的发生密切相关,通过补充维生素D可能为预防和治疗T1DM提供新的途径。

Abstract:Objective: To discuss the relationship between the level of vitamin D and some relative biochemical indexes including glycosylated hemoglobin(HbA1C),C-peptide,high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),triglyeride(TG) and total cholesterol(TC) in patients with initial type1 diabetes mellitus(T1DM).Methods:Twenty-six patients with T1DM were enrolled and twenty-four healthy children were selected as control.Levels of fasting blood glucose(FBG),TC,TG,HDL-C,LDL-C,Ca,apolipoprotein(Apo),lipoprotein-A(Lp-A) were dertermined by automatic biochemistry analyzer,HbA1C was determined by Bio-Rad glycosylated hemoglobin analyzer,fasting C-peptide was determined by electrochemiluminescence method,25(OH)D3 was determined by ELISA assay,and the relationship was analyzed.Results:There was no significant differences in TC,LDL-C,Apo-A,Apo-B,Lp-A,and Ca(P>0.05).TG,ALP,HbA1C were significantly higher(P<0.01),while 25(OH)D3,HDL-C,fasting C-peptide were significantly lower in T1DM group(P<0.01-0.05).Serum 25(OH)D3 was negatively related to TG,HbA1C,C-peptide(P<0.01-0.05),and was positively related to HDL-C(P<0.01).There was no correlation with Ca and TC.Conclusions:Vitamin D plays important role in the incidence and promotion of T1DM.Supplement of vitamin D may provide a new pathway in prevention and treatment of type 1 diabetes mellitus.
