摘    要:目的:探讨急性心肌梗死(Acute Myocardial Infarction,AMI)患者循环内皮细胞(CEC)核DNA改变的特点及动态监测copeptin含量变化的临床意义。方法:应用单细胞凝胶电泳技术(SCGE)检测60例AMI急性期(12h)患者和38例健康对照CEC核DNA的损伤;采用双抗体夹心免疫分析法测定血浆和肽素水平。结果:AMI组患者(93.33±5.75)%的CEC核DNA电泳呈明显的"彗星"状,其彗尾DNA含量为(41.12±3.77)%;对照组(10.53±3.54)%的CEC核DNA表现为"彗星"状,其彗尾DNA%含量为(7.33±2.53)%。AMI组与对照组彗星率、彗尾DNA含量相比均差异有统计学意义。(均P<0.001)。AMI组患者血浆copeptin水平显著升高,2dcopeptin含量显著升高,与对照组相比差异有统计学意义(P<0.01);12h、7dcopeptin含量明显升高,与对照组相比差异有统计学意义(P<0.05);12hcopeptin含量与7dcopeptin含量相比差异无统计学意义(P>0.05)。结论:AMI急性期患者CEC核DNA有明显损伤,动态监测copeptin水平,有利于AMI患者的治疗与预后。

Abstract:Objective: To investigate changes in nuclear DNA of circulating endothelial cells and dynamic monitoring value of copeptin in patients with acute myocardial infarction(AMI).Methods:The damage of nuclear DNA of circulating endothelial cells was assayed by single cell gel electrophoresis(SCGE)and agarose gel electrophoresis(AGE) in 60 acute-phase patients with AMI,and 38 health adults were controlled.copeptin levels in plasma were detected by sandwich immunoluminometric assay.Results:The nuclear DNA of AMI group was obviously demonstrated like comet and majority of nuclear DNA were demonstrated like circle in control group.AMI group showed a higher percentage of comet cells than the control group(93.33%±5.75% and 10.53%±3.54%,respectively,P<0.001),and their tail DNA content was significantly increased in AMI(41.12%±3.77% and 7.33%±2.53%,respectively,P<0.001).2 d Opeptin level in AMI patients was significantly higher than that of the control group(P<0.01);12 h、7 d copeptin level in AMI patients was significantly higher than control group(P<0.05);copeptin level in AMI patients was no significant difference between 12 h and 2 d(P>0.05).Conclusions: The damage of nuclear DNA for circulating endothelial cell was significant in AMI,dynamic monitor of copeptin level may contribute to the treatment or prognosis of AMI.
