骨盆原发性非霍奇金淋巴瘤的影像学表现

目的:分析骨盆原发性非霍奇金淋巴瘤的影像学表现,探讨其影像学特征及鉴别要点。方法:回顾性分析12例经病理证实的骨盆原发性非霍奇金淋巴瘤患者的影像学表现,12例均行X线检查,4例行CT检查,11例行MRI检查。结果:12例原发性淋巴瘤中,位于髂骨10例,耻骨及骶骨各1例。X线及CT主要表现为溶骨性骨质破坏8例,混合性骨质破坏4例,常合并较大软组织肿块,骶髂关节受侵犯2例,髋关节受侵犯7例;MRI检查T1WI主要表现为等信号6例,稍高信号1例,低信号4例,T2WI主要表现为以高信号为主的混杂信号,增强扫描病灶不均匀明显强化。结论:骨盆是骨原发性非霍奇金淋巴瘤的好发部位,其X线、CT表现多样,MRI信号复杂,需与多种恶性骨肿瘤鉴别,充分结合病变的临床及影像学表现,可提高病变的诊断率。

Image of primary bone non-Hodgkin's lymphoma of pelvis

Objective: To study the imaging demonstration,imaging character and differentiation of primary bone non-Hodgkin′s lymphoma of pelvis.Methods:The image demonstration of 12 cases with primary bone non-Hodgkin′s lymphoma of pelvis were retrospectively analyzed.All cases were confirmed by operation and pathological analysis.All 12 cases underwent X-ray examinations,4 cases were examined by CT and 11 cases by MRI.Results:In 12 cases with primary bone non-Hodgkin′s lymphoma of pelvis,tumors were located in lliac in 10 cases,1 cases in pubic and 1 case in sacrum,repectively.Plain film and CT revealed 8 cases with osteolytic destruction,and 4 cases with lytic mixed with irregular sclerotic.Delete soft tissue masses were found adjacent to bone destruction in all 12 patients.T1WI revealed 6 cases with isointensity signal,4 cases with hypointensity signal and 1 case with hyperintense signal.T2WI showed all cases revealed hyperintense signal and prented enhancemen after injection of contrastmedia.Conclusions:Pelvis is predilection sites of primary bone non-Hodgkin′s lymphoma.The X-ray and CT appearance of the tumor are various,and the magnetic resonance signals are complex.The tumor should be differenciated from many other malignant bone tumors.Combining the clinical and imaging can improve the diagnosis of the disease.
