老年股骨颈骨折患者治疗临床分析

目的:探讨人工股骨头置换术及空心钉内固定治疗老年股骨颈骨折患者的临床效果。方法:应用随机数字表法将2009年8月～2011年8月在我院骨科住院治疗的86例老年股骨颈骨折患者进行人工股骨头置换术及空心钉内固定治疗,对两组患者进行相应手术治疗后,采用Harris评分系统评定两组患者的髋关节功能情况。结果:进行人工股骨头置换术治疗的老年股骨颈骨折患者手术时间及术中出血量明显长于或多于进行空心钉内固定治疗组患者,差异有统计学意义(P<0.05);而进行人工股骨头置换术治疗的老年股骨颈骨折患者髋关节功能改善明显优于空心钉内固定治疗组患者,Harris评分明显也高于空心钉内固定治疗组患者的(P>0.05)。结论:人工股骨头置换术是临床治疗老年股骨颈骨折患者的一种安全有效的方法,术后老年股骨颈骨折患者髋关节功能恢复较好,值得临床推广应用。

Clinical analysis of treatment efficacy for elderly patients with transcervical fracture

Objective: To evaluate the clinical effects of artificial femoral head replacement and hollow needle internal fixation of femoral neck for elderly patients with transcervical fracture.Methods: Eighty-six elderly patients with transcervical fracture that were admitted to our hospital during August 2009 and August 2011 and treated with artificial femoral head replacement and hollow needle internal fixation of femoral neck fracture were enrolled into this study.Clinical effects of the two treamtments were evaluated and compared with Harris score system.Results:Though operation time was significant longer and blood loss volume were larger for patients underwent artificial femoral head replacement comparing with those underwent hollow needle internal fixation of femoral neck(P<0.05),they showed more obvious improvements in hip function as well as higher Harris scores.Conclusions:Artificial femoral head replacement treatment is a safe and effective treatment with good clinical effects for elderly patients with transcervical fracture,and it’s worth for clinical application in large scale.
