上颌磁力扩弓对龈沟液内AST及ALP水平的影响

摘    要:目的:研究上颌磁力扩弓(magnetic expansion,ME)前后龈沟液(gingival crevicular fluid,GCF)内天冬氨酸转氨酶(aspartatetransaminase,AST)和碱性磷酸酶(alkaline phosphatase,ALP)水平变化,探讨磁力扩弓与牙周组织改建的关系。方法:选择30例需要上颌慢速扩弓的正畸患者,随机分为对照组(戴入磁力矫治器但不加力)和实验组(持续加力1周),各15例,利用全自动化生化分析仪检测扩弓前、扩弓24h、扩弓7d及保持7、14、28d龈沟液中AST、ALP水平的变化,并对两组各治疗阶段GCF-AST、GCF-ALP的水平进行比较。结果:保持7d至保持28d,实验组GCF-AST水平高于对照组,差异有统计学意义(P<0.05~0.01);扩弓7d至保持28d,实验组GCF-ALP水平高于对照组,差异有统计学意义(P<0.05~0.01)。结论:在口腔卫生良好时,磁力扩弓组GCF-AST、GCF-ALP水平持续增高,表明磁力产生持续不断的力,在一定程度上反映其对牙周组织的改建,为正畸治疗提供参考。

Abstract:Objective:To investigate the effect of magnetic expansion(ME) on periodontal tissue by means of measuring the amount level of AST and ALP in gingival crevicular fluid(GCF),and to discuss the relationship between magnetic expansion and periodontal tissue remodeling.Methods:A total of 30 patients(aged 10.5 to 12.2 years) were randomly divided into ME group(15 cases),control group(placement of magnetic expansion appliance,but no activation,15 cases) who required opening of the palatal suture.AST and ALP activities were determined by a full automatic biochemical analyzer during 6 observation periods before activation,24 hours and 7 days after activation,7 days,14 days and 28 days in retention.Results:AST activity was statistically significant different between two groups from 7 days in retention to 28 days in retention(P<0.05-0.01).ALP activity was statistically significant different between two groups from 7 days activation to 28 days in retention(P<0.05-0.01).Conclusions:These results indicate that GCF-AST and GCF-ALP levels may reflect periodontal tissue remodeling.GCF-AST and GCF-ALP levels of ME group are continuous increased,which suggest magnetic expansion could bring continuous force.It is favorable to periodontal tissue remodeling.These results will provide the favorable reference for orthodontic treatments.
