乳房外Paget病的诊断和误诊分析

目的　探讨乳房外 Paget病的临床病理特点与误诊原因。方法　通过对 1991～ 2 0 0 1年间诊治的 33例乳房外 Paget病进行回顾性分析。结果　本病多见于老年人 ,男性多于女性。早期多为生殖器部位的湿疹样皮损。病理上 ,表皮或附属器有多少不等的 Paget细胞。在 15例当地医院误诊的病例中 ,多误诊为湿疹和 Bowen病。结论　乳房外Paget病临床表现类似于湿疹和 Bowen病 ,易误诊 ,确诊靠组织病理检查。

Analyses of Diagnosis and Misdiagnosis of Extramammary Paget's Disease

Objective To investigate the characteristic of clinicopathology and the causes of misdiagnosis of extramammary paget's disease.Methods The data of 33 patients admitted to our hospital from 1991 to 2001 were analyzed retrospectively.Results Extramammary paget's disease was more frequently seen in the aged patients,and more seen in more males than in females.It presented with eczema like lesions of the genital regions in the early stages.Histological sections showed paget cells within the epidermis or skin appendages.Among 15 cases of misdiagnostic extramammary paget's disease,they were often misdiagnosed as eczema and Bowen's disease.Conclusions The clinical manifestation of extramammary paget's disaese was similar to eczema and Bowen's disease,so it was easy to give incorret diagnosis.Biopsy must be done in order to confirm the diagnosis.
