老年患者与非老年患者原发性甲状腺功能减退症治疗后血脂变化分析

目的:分析原发性甲状腺功能减退症(甲减)老年患者与非老年患者应用左旋甲状腺素(优甲乐)治疗后血脂的变化特点。方法:原发性甲减老年患者38例,非老年患者45例在诊断初和经左旋甲状腺素治疗8周、12周、16周时分别测定空腹血清游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、高敏人促甲状腺激素(TSH)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C),分析两组对象在治疗后各周血脂变化特点。结果:优甲乐替代治疗12周后,两组患者甲状腺功能逐渐恢复正常,但两组血脂改变存在差异;12周后非老年患者TC、LDL-C均明显下降,而老年患者则要16周才明显下降;而两组TG、HDL-C在12周、16周变化均不大。结论:原发性甲减患者应用左旋甲状腺素治疗可显著降低TC、LDL-C水平;但老年性患者与非老年患者存在明显差异,12周后非老年患者TC、LDL-C均明显下降,而老年患者则要16周才明显下降。

Changes of blood lipid profile after treatment between elderly patients and non-elderly patients with primary hypothyroidism

Objective To analyze the changes of blood lipid profile after levothyroxine treatment between elderly patients and non-elderly patients with primary hypothyroidism.Method Thirty-eight elderly patients and forty-five non-elderly patients with primary hypothyroidism were recruited.All patients were examined before and 4 weeks、12 weeks、16 weeks after levothyroxine treatment.Fasting blood FT3、FT4、TSH,total cholesterol （TC） 、triglycerid （TG） 、low density lipoprotein （LDL） ,high-density lipoprotein （HDL） were determined.Analysis of two groups of objects in the characteristic of each week lipid changes after treatment.Results After levothyroxine substitutive therapy for 12 weeks,thyroid function guadually improved to normal.TC、LDL-C were significantly decreased in non-elderly patients for 12 weeks,in elderly patients for 16 weeks,There is no difference of TG or HDL-C change between two groups.Conclusion Levothyroxine treatment could result in the significantly decreaseof TC,LDL-C in patients with the primary hypothyroidism.But there is significantly different between elderly patients and non-elderly patients.The administration of levothyroxine makes TC、LDL-C significantly decreased in non-elderly patients with primary hypothyroidism after 12 weeks,but in elderly patients after 16weeks.
