子宫内膜癌患者的彩色多普勒超声参数及其与临床特征和预后的关系

目的探讨子宫内膜癌患者的彩色多普勒超声参数及其与临床特征和预后的关系。方法选择75例子宫内膜癌患者和70例子宫内膜增生患者,分别作为观察组和对照组。比较两组患者的彩色多普勒超声参数[阻力指数(RI)、搏动指数(PI)],分析不同临床特征子宫内膜癌患者的RI、PI。对子宫内膜癌患者进行2年随访,记录其预后情况,采用多元Logistic回归模型分析子宫内膜癌患者预后的影响因素。结果观察组患者的RI、PI均明显低于对照组,差异均有统计学意义(P﹤0.01)。不同年龄、组织学类型、淋巴结转移情况的子宫内膜癌患者的RI、PI比较,差异均无统计学意义(P﹥0.05)。不同临床分期、肌层浸润深度的子宫内膜癌患者的RI、PI比较,差异均有统计学意义(P﹤0.05)。Logistic回归分析结果显示,临床分期为Ⅲ~Ⅳ期、肌层浸润深度﹥1/2、RI≤0.41、PI≤0.53均是子宫内膜癌患者预后的独立危险因素(P﹤0.05)。结论彩色多普勒超声可显示子宫内膜癌患者的血流特征,为其预后评估提供可靠的影像学依据。

Color Doppler ultrasound parameters of patients with endometrial cancer and their relationship with clinical characteristics and prognosis

Objective To investigate the color Doppler ultrasound parameters of patients with endometrial cancer and their relationship with clinical characteristics and prognosis.Method A total of 75 patients with endometrial cancer and70 patients with endometrial hyperplasia were selected as the observation group and control group,respectively.The color Doppler ultrasound parameters[resistance index(RI),pulsatility index(PI)]of the two groups were compared,and the RI and PI of endometnal cancer patients with different clinical characteristics were analyzed.The patients with endometrial cancer were followed up for two years,and their prognosis was also recorded.The multivanate Logistic regression model was used to analyze the prognostic factors of endometrial cancer patients.Result The RI and PI of the observation group were significantly lower than those of the control group,and the differences were statistically significant(P<0.01).There was no statistically significant difference in RI and PI of endometrial cancer patients with different ages,histological types,and lymph node metastasis(P>0.05).The differences mRI and PI of endometnal cancer patients with different clinical stages and depth of myometrial invasion were statistically significant(P<0.05).Logistic regression analysis indicated that clinical stageⅢ-Ⅳ,depth of myometnal invasion>1/2,RI≤0.41,PI≤0.53 were independent risk factors for the prognosis of endometrial cancer patients(P<0.05).Conclusion Color Doppler ultrasound can characterize the blood flow of patients with endometrial cancer and provide robust imaging evidence for its evaluation of patients’prognosis.
