早期肠内营养支持在危重症性神经疾病患者中的应用

目的：研究神经内科ICU危重患者早期肠内营养支持的疗效。方法：将62例不能进食的危重患者随机分为早期肠内营养支持治疗组（肠内营养组）32例和匀浆膳组30例，比较入院时及2周后各项营养指标。结果：2周后，肠内营养组患者血清总蛋白（TP）、血清白蛋白（ALB）、血清前白蛋白（PA）较入院时明显增高，差异有统计学意义（P〈0．01）；匀浆膳组TP、ALB、PA较入院时增高，差异亦有统计学意义（P〈0．05或P〈0．01）。2组对比，治疗后的TP、ACB、PA在肠内营养组的改善情况均优于匀浆膳组，差异有统计学意义（P〈0．01）。匀浆膳组并发症发生率高于肠内营养组（P〈0．01）。结论：早期肠内营养支持治疗可改善危重患者的营养状况，减少并发症，改善预后。

The Clinical Research of Early Enteral Nutrition Support in Critically Ⅲ Patients with Neurological Diseases

Objective:To investigate the curative effect of early enteral nutrition support on critically ill patients in intensive care unit of department of neurology(NICU).Methods:Sixty-two critically ill patients who can not eat anything in NICU were randomly divided into enteral nutrition group and common diet group.The levels of nutritional status were measured during the periods of nutrition support.Results:After the second week of nutrition support,the levels of serum total protein(TP),albumin(ALB),prealbumin(PA) were significantly higher in enteral nutrition group than those before the enteral nutrition support(P<0.01).The levels of TP,ALB and PA were significantly higher(P<0.05 or P<0.01) in the common diet group than those before treatment.The increased levels of TP,ALB and PA were significantly higher in enteral nutrition group than those in common diet group(P<0.01).The incidence rate of complication was higher in common diet group than that of enteral nutrition group(P<0.01).Conclusion:The early enteral nutrition support might improve the nutritional condition of critically ill patients,reduce the incidence rate of complication,and promote the prognosis.
