儿童急性淋巴细胞白血病血管内皮生长因子表达与早期治疗反应的关系探讨

摘    要:目的 探讨血管内皮生长因子(VEGF)在儿童急性淋巴细胞白血病(ALL)患儿中的表达状况与临床相关危险因素及其早期治疗反应的关系。方法 采用S-P免疫组织化学方法,检测32例ALL患儿治疗前后骨髓细胞VEGF的表达。选择同期住院的非白血病患儿15例正常骨髓作为对照。结果 VEGF在ALL中的表达明显高于对照组(t=7.4917,P〈0.01),一个疗程后显著降低(t=5.7530,P〈0.01);VEGF表达治疗前后高危组、中危组、低危组差异有显著性(F=3.7781、4.2305,P均〈0.05),高危组治疗后水平下降,较化疗前差异无显著性,但显著高于对照组(t=1.9808,P〈0.05);治疗前后早期治疗反应差与早期治疗反应好者VEGF表达差异均有显著性(t=1.8439、3.4938,P均〈0.05),治疗反应好组治疗后显著降低(t=1.8038,P〈0.05),治疗反应差组较治疗前差异无显著性(t=1.3798,P〉0.05)。结论 VEGF在儿童急性白血病患者中存在高表达,与临床危险因素有关,可以作为一个影响儿童ALL疗效、预后,评定危险程度的新的生物学指标。

Abstract:Objective To explore the relationship of vascular endothelial growth factor (VEGF) with clinical risk factors and early treatment response in children with acute lymphocyte leukemia (ALL).Methods The expression of VEGF in bone marrow cells from 32 children with ALL was detected by S_P immunohistochemical staining before and after chemotherapy,bone marrow cells from 15 children without ALL served as controls. Results The level of VEGF expression in ALL group before treatment was significantly higher than that in control group (t=7.4917,P<0.01) and decreased significantly after treatment (t=5.7530, P<0.01). There were significantly differences among high risk group, moderate risk group and low risk group before and after treatment (F=3.7781,4.2305,P<0.05).The expression of VEGF in high risk group had no statistical difference before and after treatment, and was significantly higher than that in control group (t=1.9808,P<0.05). In good response group, the level of VEGF expression decreased significantly after treatment than before treatment (t=1.8038,P<0.05), but there was no diffe_rence before and after treatment in poor response group (t=1.3798,P>0.05). Conclusions The level of VEGF expression is higher and closely related to clinical risk factors and early treatment response in children with ALL.
