老年高血压病患者动态血压监测分析

目的：探讨老年高血压病（EH）患者的动态血压监测（ABPM）特点。方法：使用美国Accutraker Dx型无创性动态血压监测仪，对42例老年EH患者、38例老年健康者及39例非老年EH患者进行24h动态血压监测。然后，对所得数据进行统计学分析。结果：老年EH患者24h、日间、夜间平均收缩压和舒张压均明显增高，以收缩期高血压为主要表现；24h、日间、夜间收缩压和舒张压负荷也明显增高，仍以收缩期血压负荷增加为主要表现；出现昼夜节律性紊乱，低谷及勺型发生率明显降低（分别为40．48％和26．19％）。结论：平均动脉压和血压负荷可做为高血压诊断的重要参数；血压昼夜节律紊乱及勺型发生率减低可做为高血压预后判断的重要参数。

Analysis on Ambulatory Blood Pressure Monitoring in Aged Patients With Essential Hypertension

Objective:To evaluate the features of ambulatory blood pressure monitoring (ABPM) in aged patients with essential hypertension(EH). Methods: Noninvasive blood pressure recorder (Accutraker Dx) was used to carry out 24 hour ABPM in 42 aged patients with EH, 38 aged controls and 39 nonaged patients with EH. The statistical analyses were done with the obtained readings. Results: 24 hour systolic blood pressure (SBP) and dystolic blood pressure (DBP),mean daytime SBP and DBP, mean night time SBP and DBP were increased in aged patients with EH. Especially, systolic hypertension was the main character,24-hour SBP and DBP load, daytime SBP and DBP load, nighttime SBP and DBP load were also increased in aged patients with EH. Still, the increase of SBP load was the main character. In addition, the blood pressure day-night regularity and variability disordered in aged patients with EH, the incidence of the deep valley and ladle-type decreased (by 40.48% and 26.19% respectively). Conclusion: The changes of the mean BP and the blood pressure load were important parameter for prognosis of hypertension. The blood pressure day-night regularity and variability shapes were important parameter of diagnosis of hypertension.
