超声在盆腔炎性肿块与卵巢肿瘤鉴别诊断中的应用

目的：探讨超声对女性盆腔炎性肿块和卵巢肿瘤的鉴别诊断价值。方法：对155例盆腔肿块超声检查的资料分组进行统计学分析。重点分析10项声像图特征。结果：对比组间在病灶内正常卵巢结构、盆腔带状回声、囊腔内实性区、囊液透声性、壁厚度、边界不清、不规则、腹水量方面差异有显著性意义；在囊壁乳头结构、囊内分隔方面差异无显著性意义。结论：病灶内发现正常卵巢结构是盆腔脓肿及炎性肿块区别于卵巢癌的重要声像图鉴别点；囊液透声差、壁增厚、盆腔带状回声有助于盆腔脓肿及炎性肿块的确诊。

Sonographic Differential Diagnosis of Pelvic Inflammatory Mass and Ovarian Tumors

Objective: To investigate the value of ultrasonography in differential diagnosis of female pelvic inflammatory masses and ovarian tumors. Methods: Sonographic findings of 155 patients with pelvic masses consisting of 4 groups were reviewed) statistically analysed and compared. Emphases lay on analysing the 10 items of ultrasonographic characteristics. Results: Eight of 10 sonographic features were found to have statistically significance among the four groups, that is, normal ovarian tissue in the lesion, band-like tissue from the pelvis , solid areas within the cysts, echogenicity of cyst, the wall thickness and irregular or poor borderline and the ascitic quantity. No significant difference was found in the septa or the nodular projection from the cyst wall. Conclusions : Appearance of normal ovarian tissue in the lesion is a specific finding in differentiating of an adnexal inflammatory mass or abscess from a malignant tumor. Poor echogenicity of the cystic cavity, the thickened wall and the banded echo from the pelvis are useful features in diagnosing pelvic inflammatory masses and abscesses.
