儿童传染性单核细胞增多症临床特点与发病年龄的关系(附142例分析)

目的　探讨儿童传染性单核细胞增多症(infectiousmononucleosis ,IM )临床特点与发病年龄的关系。方法　将1 4 2例儿童IM分成0～3岁,>3～6岁,>6～1 5岁3个组,进行回顾性分析。结果　儿童IM 6岁以下儿童占66.9% ,临床表现以发热、咽峡炎、淋巴结肿大、肝脾大为主。年长儿发热时间长,眼睑浮肿和皮疹在年幼儿多见。外周血白细胞年幼儿升高明显,异型淋巴细胞年幼儿偏低,血清ALT、AST值在年长儿偏高,血清LDH值在各年龄组均呈高值。血清嗜异性凝集试验年长儿阳性率高。结论　儿童IM某些临床特点与发病年龄有关,应予以重视,以提高确诊率。

Relation between clinical features and age in 142 children with in fectious mononucleosis cases reported

Objective To explore the clinical features of childhood infectious mononucleosis in relation to age of the children. Methods 142 pediatric patients with IM were separated into three age groups of 0 to 3 years, >3 to 6 years, >6 to 15 years, and were analyzed retrospectively. Result Childhood IM mostly occurred at under 6 years old, accounted to 66.9%. The majority of presentations were fever, angina, lymphnodes enlarged and hepatosplenomegaly. Duration of fever was longer in the older age group. Eruption and eyelids edema were more often detected in the younger age group. The leukocyte count was higher in the younger age group. The serum ALT and AST levels were higher in the older age group. AL was lower in the younger age group. The serum LDH levels were high in all age groups. The positive rates of Paul-Bunnell test, is higher in the older age group. Conclusion The clinical feature of childhood IM varied according to the age. We must consider the age of IM children will further increase the accuracy of diagnosis of IM.
