基层医院乳腺癌筛查模式的研究

目的探讨基层医院乳腺癌筛查的优化模式。方法对1998年1月至2006年12月新会地区的47976名妇女进行乳腺保健教育实施的乳腺临床检查加乳腺B超检查的乳腺癌筛查模式进行分析和总结。结果47976名妇女中筛查出乳腺异常情况者12694人,其中1126人接受手术活检或空心针穿刺活检,确诊为乳腺良性病变1069例,占94.1%,确诊为乳腺癌57例,占5.1%,其中非浸润性癌7例（均为T0期）,包括有小叶原位癌3例,导管内癌4例;浸润性癌50例,其中T1期是16例。本组乳腺癌筛查的乳腺癌检出率为0.12%。结论临床乳腺检查加乳腺B超检查的乳腺癌筛查模式能提高基层医院的乳腺癌早期诊断水平,有一定的临床推广价值。

Study on the screening model for breast cancer in grass-roots hospital

Objective To discuss the optimal screening model for breast cancer in grass-roots hospital. Methods From January 1998 to December 2006,47976 women of Xinhui District received scrreening model for breast cancer with breast clinical examination plus breast ultrasonography were analyzed and summarized. Results Among the 47976 women, abnormal status of breast was screened in 12694 cases. Among them, 1126 cases underwent operative biopsy or core-needle biopsy. It was confirmed by pathology that 1069 cases were benign lesions(94.10%),57 cases were breast cancer(28.1%). In the latter, 7 cases were belonged to non-infiltrated cancer (To stage), including lobular in situ cancer 3 cases and intra intra-ductal cancer 4 cases. And 50 cases were belonged to infitrated cancer and among them 16 cases were in T1 stage. In the screening of breast cancer of this group, the detectable rate of breast cancer was 0.12%. Conclusion Screening model of breast cancer of clinical breast examination plus breast ultrasonography can improve the level of early diagnosis of breast cancer in grass-roots hospital. It has a definite value in the clinical application.
