超声引导结合改良Seldinger技术PICC置人法在肠外营养患者中的应用

目的：分析超声引导结合改良Seldinger技术PICC置入法在肠外营养患者中的应用效果。方法：选取40例需肠外营养治疗的患者实施PICC留置管，随机分为观察组和对照组各20例，观察组采用超声引导下结合改良Seldinger技术P1CC置入法，对照组采用PICC盲穿置入法。结果：两组患者置管后静脉炎、感染、导管堵塞、置管天数、日均耗材费用差异无统计学意义（P〉0．05），但24h后穿刺点渗血发生情况、置管期间维护次数两组差异有统计学意义（P〈0．05）。结论：超声引导下结合改良Seldinger技术PICC置人法能改善穿刺点渗血，从而减少维护次数，不增加患者经济负担。

Application of ultrasound guided combined with modified Seldinger method PICC placement in patients with parenteral nutrition

Objective：To analyze the application efficacies of upper ann placement of peripherally inserted central venous catheters using ultrasound guid- ance and modified Seldinger technique with traditional method, in patients who received parenteral nutrition. Methods：A total of 40 PICC patients who will receive pareteral nutrition were divided into the observational group （n = 20） and control group （n = 20）. The patients of the observational group were trea- ted with P1CC puncture using ultrasound guidance and modified Seldinger technique. While the control group were treated with traditional method. Results ： No significant differences were found on the incidence of phlebitis, infection, catheter obstruction average days of placed catheter and the supplies cost（P 〉 0, 05）, but there was significant differences on incidence of hemorrhage of insertion sites and maintenance numbers（ P 〈 0.05）, Conclusion：The method of PICC insertion utilizing ultrasound guidance and modified Seldinger technique can reduce the incidence of hemorrhage of insertion sites, maintenance numbers and can not increase economic burden.
