摘    要:目的:分析超声引导结合改良Seldinger技术PICC置入法在肠外营养患者中的应用效果。方法:选取40例需肠外营养治疗的患者实施PICC留置管,随机分为观察组和对照组各20例,观察组采用超声引导下结合改良Seldinger技术P1CC置入法,对照组采用PICC盲穿置入法。结果:两组患者置管后静脉炎、感染、导管堵塞、置管天数、日均耗材费用差异无统计学意义(P〉0.05),但24h后穿刺点渗血发生情况、置管期间维护次数两组差异有统计学意义(P〈0.05)。结论:超声引导下结合改良Seldinger技术PICC置人法能改善穿刺点渗血,从而减少维护次数,不增加患者经济负担。

Abstract:Objective:To analyze the application efficacies of upper ann placement of peripherally inserted central venous catheters using ultrasound guid- ance and modified Seldinger technique with traditional method, in patients who received parenteral nutrition. Methods:A total of 40 PICC patients who will receive pareteral nutrition were divided into the observational group (n = 20) and control group (n = 20). The patients of the observational group were trea- ted with P1CC puncture using ultrasound guidance and modified Seldinger technique. While the control group were treated with traditional method. Results : No significant differences were found on the incidence of phlebitis, infection, catheter obstruction average days of placed catheter and the supplies cost(P 〉 0, 05), but there was significant differences on incidence of hemorrhage of insertion sites and maintenance numbers( P 〈 0.05), Conclusion:The method of PICC insertion utilizing ultrasound guidance and modified Seldinger technique can reduce the incidence of hemorrhage of insertion sites, maintenance numbers and can not increase economic burden.
