| 摘 要： | 目的探讨右美托咪定(dexmedetomidine, Dex)复合舒芬太尼用于妇科腹腔镜术后镇痛对患者睡眠质量的影响。 方法选择全身麻醉下拟行妇科腹腔镜手术的患者60例,采用随机数字表法分为舒芬太尼组(A组)、Dex复合舒芬太尼组(B组),每组30例。术后患者自控静脉镇痛,A组给予舒芬太尼2.0 μg/kg,B组给予舒芬太尼2.0 μg/kg+Dex 200 μg/kg。记录并分析两组VAS评分、拉姆齐镇静评分(Ramsay Sedation Scale, RSS)、舒芬太尼累积消耗量、 Medicaoutcomes Study Sleep Scale(MOS-SS)评分、主观睡眠质量评分及睡眠障碍指数(sleep problem index, SPI)。 结果与A组比较:B组患者术后6、12、24、48 h VAS评分显著降低,舒芬太尼累积消耗量显著减少,RSS评分显著升高,差异有统计学意义(P<0.05);B组患者手术前夜主观睡眠质量评分与术前1周平均每日睡眠时间、SPI差异均无统计学意义(P>0.05);B组术后第1夜、术后第2夜主观睡眠质量评分显著升高,术后1周平均每日睡眠时间显著增长,SPI显著减小,差异有统计学意义(P<0.05)。 结论Dex复合舒芬太尼用于妇科腹腔镜手术患者术后镇痛效果较好,且能显著改善患者术后睡眠质量,为术后快速康复提供有利条件。
