COPD患者血清淀粉样蛋白A变化及意义

目的探讨慢性阻塞性肺疾病（COPD）患者中血清淀粉样蛋白A（SAA）水平的变化及意义。方法抽取COPD急性期、缓解期患者及健康对照者静脉血,ELISA法检测血清中SAA、CRP、TNF-a及IL-8水平。结果COPD急性期组SAA和CRP水平显著高于缓解期及对照组（P〈0.01）,缓解期组SAA水平显著高于对照组（P〈0.01）;血清SAA水平与TNF-a、IL-8均相关）r=0.76;r=0.71）,血清CRP水平与TNF-a、IL-8均相关（r=0.69;r=0.63）;血清SAA诊断AECOPD的ROC曲线下面积为0.837±0.066,CRP的ROC曲线下面积为0.751±0.059。结论血清SAA对COPD的临床分期具有较高的应用价值,并可能为AECOPD发病机制提供新的见解。

Changes in serum amyloid A level and its clinical significance in patients with chronic obstructive pulmonary disease

Objective To explore the changes and clinical significance of serum amyloid A(SAA)levels in patients with chronic obstructive pulmonary disease(C0PD).Methods Blood samples in patients with COPD both during acute exacerbation and stable stage were collected.Healthy volunteers were enrolled as controls.The levels of serum SAA、CRP、TNF-a and IL-8 were measured by ELISA.Results The 1evels of SAA and CRP in patients with AECOPD were significantly higher than those in patients with COPD during stable stage and in the controls(P<0.01).The levels of SAA in patients with COPD during stable stage were significantly higher than those in the controls(P<0.01);serum SAA levels had significantly correlations with TNF-a and IL-8(r=0.76;r=0.71),serum CRP levels had significantly correlations with TNF-a and IL-8(r =0.69;r=0.63);The areas under R0C curve of serum SAA to diagnose AECOPD was 0.837±0.066,those of serum CRP was 0.751±0.049.Conclusion SAA could take part in the pathogenesis of COPD and have higher value in clinical application for the differential stage of COPD.
