家庭医生签约服务在老年高血压患者社区护理管理中的应用价值

目的:探讨家庭医生签约服务在老年高血压患者社区护理管理中的应用价值。方法:选取2020年1—12月上海市闵行区颛桥社区卫生服务中心收治的200例老年高血压患者作为签约组,另选取同期该社区医院收治的150例未签约老年高血压患者作为对照组。对照组实施常规护理管理,签约组在对照组常规护理的基础上融入家庭医生签约服务,比较两组患者血压控制、服药依从性以及自护能力情况。结果:干预3个月后,两组Morisky用药依从性问卷评分均存在不同程度增长,但签约组明显高于对照组。两组自我护理能力量表评分均存在不同程度上升,但签约组明显高于对照组。两组患者收缩压和舒张压水平均存在不同程度下降,但签约组明显低于对照组。结论:老年高血压患者社区护理管理过程中融入家庭医生签约服务能够改善其服药依从性和自护能力,进一步稳定患者血压水平。

Application Value of Family Doctor Contract Service in the Community Care Management of Elderly Hypertension Patients

OBJECTIVE To explore the application value of family doctor contract service in the community care management of elderly hypertensive patients.METHODS 200 elderly patients with hypertension received in the community hospital in December 2020 were selected as contracted group,and 150 elderly patients with uncontracted hypertension received by our community hospital in the same period were taken as control group.The control group carried out routine community care management,and the contracted group integrated the family doctor contract service on the above basis and compared the blood pressure control,medication compliance and self-protection ability of the two groups.RESULTS After 3 months of intervention,Morisky increased compliance scores,but significantly higher than controls,ESCA scores,but significantly higher than controls.SBP and DBP levels decreased differently in both groups,but the contracted group were significantly lower than in the control group.CONCLUSION Signing service of family doctors in the community care management of elderly hypertensive patients could improve their medication compliance and self-care ability and further stabilize the patient blood pressure level.
