Clinical Value of Special Position Selection in Mammography for the Detection of Lesions in the Posterior Band of Breast

Abstract:Objective To explore the application value of the special position of mammography combined with the conventional standard position in the detection of lesions in the posterior band of breast. Methods We collected 49 subjects’ image data who underwent mammography of conventional standard position and special position from July 2019 to July 2020.Retrospective analysis was performed of the influence on the detection rate, edge, size display of posterior breast lesions optimized conventional standard position special position and the combination of the both. Results Suspicious lesions were found in 49 breasts of 49 subjects, all of whom were female. The lesions were located in the upper posterior band of the breast in 9 cases, the lower posterior band of the breast in 7 cases, the upper inner quadrant posterior band of the breast in 13 cases, the lower inner quadrant posterior band of the breast in 9 cases, and the inner side posterior band of the breast in 11 cases. Among them, 30 cases-Mediolateral/Lateromedial(ML/LM),13 cases selected the Exaggerated CranioCandal(XCC) view.2 cases-the light pressure CC view, 3 cases-Lateral Medial Oblique(LMO) view, and 1 case-the Cleavage view.Average glandular dose(AGD) was about 2.03 mGy in the conventional position and 1.00 mGy in the special position, with a total dose increase of about 10.96%.The image quality of all image met the requirements of clinical diagnosis. The detection rate increased from 83.7% to 100%.The rate of clear lesion edge display reaching clinical diagnosis increased from 53.1% to 96.0%. The detection rate of lesion size increased from 38.7% to 83.7%. Conclusion Adding a special positioning improves the image quality, detection rate, edge and size display degree of the posterior third of the breast lesions has obvious advantages compared with the conventional radiographic position alone, which significantly improves the clinical diagnosis of mammography.
